Summary: The study established a nomogram for predicting lymph node metastasis in early gastric cancer, with age, tumor size, submucosal invasion, histological subtype, and HER2 positivity identified as independent risk factors. The model showed good performance in both internal and external validation, with the prediction model providing a higher net benefit for guiding treatment.

Summary: Endoscopic surgery is commonly used for EGC treatment, but LNM remains a significant risk factor for relapse. The study found that GNE expression is down-regulated in EGC patients with LNM. Combining GNE expression with other predictive factors can improve the accuracy of LNM prediction in EGC patients.

Summary: The study proposes a deep-learning framework to analyze lymph node whole-slide images, identifying lymph nodes and tumor regions, and revealing the importance of the tumor-area-to-MLN-area ratio (T/MLN) in predicting the prognosis of gastric cancer patients.

Summary: Metabolic reprogramming is a hallmark of malignancies such as gastric cancer, with the rate-limiting enzyme SOAT1 identified as highly expressed in cancerous tissues and associated with advanced tumor stage and lymph node metastasis. Inhibition of SOAT1 suppresses GC cell proliferation, cholesterol ester synthesis, and lymphangiogenesis, while overexpression promotes these processes by regulating cholesterol metabolism genes. Therefore, SOAT1 may serve as a promising prognostic biomarker for guiding clinical management and treatment decisions in gastric cancer.

Summary: This study identified specific methylation signatures for lymph node metastasis (LNM) in early gastric cancer (EGC) and established a 3-marker methylation model with diagnostic potential to accurately identify LNM, thus reducing unnecessary gastrectomy in EGC patients.

Summary: This study investigated lymph node metastasis (LNM) and its impact on overall survival (OS) in Western patients with early gastric cancer (EGC). The study found a higher rate of lymph node positivity, suggesting caution in applying expanded criteria for endoscopic resection.

Summary: Tumor budding is associated with tumor size, tumor histologic differentiation, depth of invasion, lymph node metastasis, lympho-vascular invasion, and early recurrence of early gastric cancer. High-grade tumor budding, especially, can predict lymph node metastasis in early gastric cancer and may be an important parameter for evaluating the prognosis of patients with early gastric cancer.

Summary: In a large national cohort in the US, factors such as lymphovascular invasion, high-grade histology, T stage, and tumor size were independently associated with lymph node metastasis in early gastric adenocarcinoma. Patients with low-grade tumors, smaller than 3 cm, and without lymphovascular invasion had a very low risk of nodal involvement, suggesting they may be considered for endoscopic resection.

Summary: The purpose of this study was to develop and evaluate a CT-based model using deep learning features to predict lymph node metastasis in locally advanced gastric cancer (LAGC). The results showed that the model had efficient and accurate predictive performance, which could benefit the clinical decision-making process for therapeutic strategy.

Summary: The study comprehensively analyzed early-onset early-stage gastric cancer, identifying intestinal type as a risk factor for lymph node metastasis and survival compared to the diffuse type. The oncogenic expression promoting the occurrence of LNM was different between intestinal and diffuse types, suggesting clinicians should focus more attention on intestinal-type EEGC.

Summary: The study aimed to clarify the clinicopathological characteristics and prognostic relevance of extra-nodal metastasis (ENM) in gastric cancer patients undergoing curative gastrectomy. Results showed that ENM was an independent prognostic factor with a prognostic significance different from that of lymph node metastasis.

Summary: This study aimed to evaluate the diagnostic value of CT in detecting lymph node metastasis in patients with early gastric cancer. The study found that the number, sum of long diameter, and sum of short diameter of lymph nodes greater than 3 mm on CT images were preferable indicators for diagnosing LNM in EGC patients. Different regional lymph nodes had different optimal criteria for predicting LNM.

Summary: A retrospective study of 1038 patients with proximal advanced gastric cancer undergoing total gastrectomy found that prophylactic splenic hilar lymph node dissection without splenectomy did not improve long-term survival outcomes. Therefore, this procedure may not be essential for patients with proximal advanced gastric cancer as well as those with greater curvature involvement or Borrmann type IV disease.
