Sinusal localization of nodal micrometastases is a prognostic factor in breast cancer

Background: Breast cancer micrometastases are frequently found during pathological examination of sentinel lymph nodes and complete axillary lymph node dissection. Despite this, their clinical relevance is still debated. The aim of this study is to investigate features that affect disease-free survival (DFS) and overall survival (OS) in patients with nodal micrometastases from breast cancer. Material and methods: We retrospectively investigated the outcome of 122 patients with nodal micrometastases from breast cancer followed up for 60 months. Results: At univariate analysis, worse DFS was related to features of primary tumor (multifocality P = 0.002; size > 2 cm, P = 0.022; grade P = 0.022; absence of estrogen P < 0.001 and progesterone P < 0.001 receptors; HER-2 overexpression P = 0.006; vascular invasion P = 0.039; proliferative fraction >= 20% P = 0.034) and micrometastases (sinusal localization P = 0.010). Among the above-mentioned features, two were strongly associated with worse DFS in the multivariate model, i.e. negative receptorial status [hazard ratio (HR) = 11.24, 95% confidence interval (CI) 4.06-31.09; P < 0.001] and sinusal localization of micrometastasis (HR = 3.66, 1.18-11.36; P = 0.025). The OS was influenced by multifocality (P < 0.001) and receptor status (P = 0.005). Conclusion: Our results indicate that in patients affected by breast cancer, in addition to the well-known pathological features of primary tumor, sinusal localization of micrometastasis strongly impacts on the prognosis.

Summary: The study indicates a crucial role of SerpinB3 (SB3) in mediating the malignant phenotype of cholangiocarcinoma (CCA) and identifies it as a new therapeutic target. Overexpression of SB3 in the stem-like subset of tumor cells is associated with enhanced sphere-forming capacity, invasion ability, and poor prognosis in both mouse models and human CCA patients. These findings suggest SB3 as a potential target for future CCA therapies.

Summary: This study identified two immune subtypes of combined hepatocellular-cholangiocarcinoma (cHCC-CCA): an immune-high subtype characterized by higher densities of immune cells and activation of gene signatures related to immune response and immunotherapy in hepatocellular carcinoma (HCC), and an immune-low subtype. The immune-high subtype was associated with improved overall survival. These findings suggest that a subset of cHCC-CCA patients may benefit from immunomodulating therapeutic approaches.

Quantitative Analysis of the Histological Features of Tuberous Breast

Summary: This study used histological specimens and digital pathology software to analyze the differences in collagen fibers and elastic fibers in patients with tuberous breast deformity (TB) compared to normal breasts. The results showed increased concentrations of collagen fibers and decreased elastic fibers in TB, highlighting the correlation between abnormal extracellular matrix concentrations and the characteristic stenotic and less elastic morphology of the deformity.

Summary: This study found that aberrant activation of Hedgehog signaling pathway was implicated in intrahepatic cholangiocarcinoma (iCCA), and targeting the main transducer Smoothened (SMO) and transcription factors GLI1 and GLI2 could inhibit the growth and survival of iCCA cells. Pharmacologic inhibition of SMO led to DNA damage, mitotic arrest and apoptotic cell death in iCCA cells. Additionally, SMO inhibition activated the DNA damage kinase WEE1, increasing the vulnerability to WEE1 inhibition.
