Summary: This study evaluated the role of ST-segment resolution (STR) and Thrombolysis in Myocardial Infarction (TIMI) flow in reperfusion evaluation after primary percutaneous coronary intervention (PPCI) for ST-segment-elevation myocardial infarction. The results showed that STR =50% and complete STR were significantly associated with lower 2-year mortality compared to STR <50%. Additionally, the combination of STR and TIMI flow provided incremental prognostic information beyond either measure alone.

Summary: In this meta-analysis of 23 studies, sacubitril-valsartan was found to improve left ventricular ejection fraction, left ventricular end-diastolic diameter, and left ventricular end-diastolic volume in patients with acute STEMI within 6 months after surgery. These results suggest that sacubitril-valsartan can reduce reperfusion injury and improve cardiac function.

Summary: Using a new-generation drug-eluting stent, the study compared the 2-year clinical outcomes of patients with diabetes mellitus (DM) and non-DM concomitant with a non-ST-segment elevation myocardial infarction (NSTEMI) and an ST-segment elevation myocardial infarction (STEMI) who underwent percutaneous coronary intervention. The results showed higher non-CD rates in patients with DM and higher CD rates in patients without DM in the NSTEMI group.

Summary: This study assessed redundant randomized clinical trials (RCTs) on ST segment elevation myocardial infarction (STEMI) patients in China and the USA and estimated the harm caused to enrolled patients. The results showed that out of 983 eligible RCTs conducted in mainland China, 775 (78.8%) were redundant and not referenced by clinical practice guidelines. The control groups of these redundant RCTs experienced 3305 extra major adverse cardiac events (MACEs), indicating potential research waste and violation of research ethics.

Summary: This study compared the attenuated ST-segment elevation over repetitive ischemic preconditioning (IPC) cycles in Gottingen and Ossabaw minipigs. The results showed that while IPC still attenuated ST-segment elevation in Ossabaw minipigs, it failed to reduce infarct size compared to Gottingen minipigs. This suggests that the block of IPC signal transduction in Ossabaw minipigs occurs distal to the sarcolemma.

Summary: There is no consensus on the optimal stent implantation parameters that enhance clinical outcome during percutaneous coronary interventions (PCIs) using optical coherence tomography (OCT) guidance. This study retrospectively examined the predictive accuracy of different suboptimal stent implantation definitions for long-term risk of device-oriented cardiovascular events (DoCE). The results showed that the definitions proposed by CLI-OPCI II and ILUMIEN-IV OPTIMAL PCI studies were predictive of DoCE, while the FORZA trial definition was not.

The aim of this narrative review is to emphasize the importance of assessing frailty in guiding therapeutic decision-making and tailoring coronary artery disease management to the specific needs of older adults, taking into account age-related changes and potential drug interactions. The review also discusses gaps in evidence and offers perspectives on optimizing coronary artery disease management in older adults.
