1. The Rotterdam classification should be used to define PCOS in the event of: menstrual cycle anomalies; amenorrhoea, oligomenorrhoea or long cycles, clinical and/or biochemical hyperandrogenism and ultrasound appearance of polycystic ovaries. 2. The presence of two of these three criteria is sufficient once all other diagnoses have been ruled out. 3. Diagnosis of hirsutism Should not be based on the Ferriman-Gallway score. 4. The ultrasound definition of PCOS contains precise criteria that must be included in the report: presence of at least 12 follicles in each ovary measuring 2-9 mm in diameter, and/or increase in ovary size > 10 ml. 5. Screening for elevated plasma LH no longer necessary. Testing for GnRH serves no purpose. 6. Routine screening for metabolic abnormalities should be carried out systematically based on weight, height and BMI, waist circumference, blood pressure and laboratory parameters: plasma glucose, triglycerides, HDL cholesterol. 7. In the case of obesity (BMI > 30 kg/m(2)), oral glucose tolerance testing (OGTT) is recommended where fasting serum glucose is normal. 8. Clomiphene citrate (CC) remains the first-line therapy for ovulation induction. In patients with BMI > 30, it should be preceded by improvement of metabolic status through appropriate lifestyle modifications. (C) 2009 Elsevier Masson SAS. All rights reserved.

Comparison of two endometrial preparation methods for frozen-thawed embryo transfer in anovulatory PCOS patients: Impact on miscarriage rate

Summary: This study evaluated the impact of endometrial preparation on FET outcomes in anovulatory PCOS patients. Results showed that early pregnancy rates, miscarriage rates, and live birth rates were similar between stimulated and artificial cycles. In conclusion, the type of endometrial preparation does not influence FET outcomes, specifically regarding the miscarriage rate, in anovulatory PCOS women.

Summary: The study aimed to investigate the association between serum LH levels in women with FHA and PCOM and BMI, serum insulin, and AMH levels using a larger population of FHA. The results showed that 42% of the FHA population had PCOM, and the PCOM+ group had significantly higher BMI and serum AMH levels and significantly lower serum FSH levels compared to the other group. LH was positively correlated with fasting insulin and AMH in the PCOM+ group only, but not with BMI.

Summary: This study investigated the changes in serum concentration of total and cleaved anti-Muullerian hormone (AMH) molecular forms and androgens in women with and without PCOS before and throughout pregnancy. The results showed that women with PCOS had higher levels of total and cleaved AMH compared to controls from preconception to the third trimester of pregnancy, and higher levels of testosterone and androstenedione during pregnancy.

Outcome of Gastric Bypass Surgery on Patients with Polycystic Ovary Syndrome: A Review

Summary: Polycystic ovary syndrome (PCOS), the most common endocrine disorder in women of reproductive age, is associated with obesity. The most effective method to achieve and maintain long-term weight loss is by the Roux-en-Y gastric bypass (RYGB). RYGB leads to adequate weight loss and reduction in BMI in obese PCOS patients, as well as improvements in testosterone levels, hirsutism, and cycle irregularities. However, more comprehensive studies are needed to evaluate fertility outcomes in this patient population.

Using serum anti-Mullerian hormone levels to predict the chance of live birth after spontaneous or assisted conception: a systematic review and meta-analysis

Summary: This study conducted a systematic review and meta-analysis to evaluate whether serum anti-Mullerian hormone (AMH) level can predict the cumulative live birth (CLB) rate after assisted reproductive technology (ART) or in women trying to conceive naturally. The results showed a positive relation between serum AMH level and CLB after IVF/ICSI, but there is a lack of data for IUI or natural conception without ART.

Summary: The objective of this study was to identify clinical, biological, and ultrasound factors that may predict resistance to 50 mg/day of CC in the first cycle of treatment for women with anovulatory PCOS. The study found that higher levels of AMH and lower levels of SHBG were significantly associated with resistance to 50 mg/day of CC. However, no satisfactory thresholds have been established for predicting resistance to 50 mg CC.
