Guillain-Barre syndrome following chickenpox: a case series

Guillain-Barre syndrome (GBS) is an acute, immune-mediated polyradiculoneuropathy, usually triggered by an infectious episode, mostly of viral origin. Varicella zoster virus (VZV) is a rare cause of GBS, mainly in the case of latent infection reactivation. We report on three adult patients who developed GBS following chickenpox, after a short period of latency. They were promptly treated with intravenous immunoglobulin, and the first one with plasma exchange additionally. All the patients experienced almost complete clinical recovery. Our experience suggests that primary VZV infection constitutes a GBS triggering event.

Summary: This study investigates the impact of misglycosylation on the functionality of P0, the major structural glycoprotein of peripheral nerve myelin. Through the use of CRISPR/Cas9, a mouse model carrying the MPZ(D61N) mutation was generated to mimic the severe early-onset form of CMT1B seen in humans. The findings suggest that hyperglycosylation interferes with myelin formation and leads to motor impairment and other pathological changes.

Summary: This study investigated fatigue in CMT patients and found that 36% of the patients experienced abnormal fatigue. Abnormal fatigue was correlated with disease severity, anxiety, depression, sleepiness, and obesity. Therefore, the management of CMT patients should involve treating fatigue and addressing other factors such as anxiety, depression, sleepiness, and obesity.

Summary: We developed a novel unsupervised cluster analysis method to study 600 patients with suspected inherited myopathy. By integrating genetic, morphological, and clinical data, we identified 4 clusters with distinctive clinical and morphological features among 64 patients with RYR1 variants. This study provides more precise genotype-phenotype correlations for RYR1-related myopathy.

Summary: Data from the Italian CMT Registry show that 1012 patients were registered, with 535 females, and 711 patients received a genetic diagnosis. The most common mutation was the PMP22 duplication (45.2%). CMT4A was the most severe type, followed by CMT4C and CMT1E. Foot deformities and walking difficulties were the main features. Shoe inserts and orthotic aids were used by almost one-half of all patients. Scoliosis was present in 20% of patients, especially in CMT4C.

Summary: This study evaluated the outcomes of COVID-19 in myasthenia gravis (MG) patients before and after vaccination. The severity of the previous MG course and during SARS-CoV-2 infection was comparable between vaccinated and non-vaccinated patients. Non-vaccinated patients had higher hospitalization, severe course, and mortality rates compared to vaccinated patients.
