Ambulatory continuous peripheral nerve blocks to treat postamputation phantom limb pain: a multicenter, randomized, quadruple-masked, placebo-controlled clinical trial

Brian M. Ilfeld, Bahareh Khatibi, Kamal Maheshwari, Sarah J. Madison, Wael Ali Sakr Esa, Edward R. Mariano, Michael L. Kent, Steven Hanling, Daniel Sessler, James C. Eisenach, Steven P. Cohen, Edward J. Mascha, Chao Ma, Jennifer A. Padwal, Alparslan Turan

Summary: The study found that a 6-day continuous peripheral nerve block can reduce phantom limb pain and improve physical and emotional dysfunction in patients for at least 1 month. Patients who received local anesthesia showed better improvement in overall impression of change and reduction in pain-induced physical and emotional dysfunction, but not in depression scores.

Molecular Modification of Transient Receptor Potential Canonical 6 Channels Modulates Calcium Dyshomeostasis in a Mouse Model Relevant to Malignant Hyperthermia

Brian M. Ilfeld, James C. Eisenach, Rodney A. Gabriel

Summary: The study demonstrates that overexpressing a nonconducting TRPC6 channel can partially ameliorate the imbalance of calcium and sodium in muscle caused by RYR1 variants, prolonging survival time during halothane exposure, but is not sufficient to prevent the lethal effects of exposure to volatile anesthetic malignant hyperthermia-triggering agents.

Percutaneous Peripheral Nerve Stimulation (Neuromodulation) for Postoperative Pain: A Randomized, Sham-controlled Pilot Study

Summary: Percutaneous peripheral nerve stimulation reduced pain scores and opioid requirements without systemic side effects during at least the initial week after ambulatory orthopedic surgery. This study helps pave the way for future clinical trials.

Oshinsky, Friedhelm Sandbrink, Joachim Scholz, Richard Scranton, Lee S. Simon, Deborah Steiner, Kenneth Verburg, Ajay D. Wasan, Kerry Wentworth

Summary: Randomized clinical trials have shown the efficacy of opioid analgesics in pain management, but they also come with significant risks. Efforts to reduce their use are ongoing, with IMMPACT providing recommendations for opioid-sparing clinical trial design. These recommendations aim to guide the design, conduct, analysis, and interpretation of future trials.

A Painful Beginning: Early Life Surgery Produces Long-Term Behavioral Disruption in the Rat

Douglas G. Ririe, James C. Eisenach, Thomas J. Martin

Summary: Early life surgery can have long-term effects on anxiety, attentional dysfunction, and opioid reward behaviors in rats, with increased anxiety, impaired attention with audiovisual distraction, and greater opioid intake observed in animals subjected to surgery.

Summary: Continuous peripheral nerve block can alleviate phantom and residual limb pain during the infusion period, but few improvements were observed until 28 days after catheter removal.

Percutaneous Neuromodulation of the Brachial Plexus and Sciatic Nerve for the Treatment of Acute Pain Following Surgery: Secondary Outcomes From a Multicenter, Randomized, Controlled Pilot Study

Summary: The study shows that ambulatory percutaneous PNS for brachial plexus and sciatic nerve leads significantly reduces postoperative pain and opioid consumption. This treatment approach demonstrates effectiveness in acute pain management following orthopedic surgery, without systemic side effects.

Preliminary results from a randomized, controlled, cross-over trial of intrathecal oxytocin for neuropathic pain

James C. Eisenach, Regina S. Curry, Timothy T. Houle

Summary: This study compared the effects of intrathecal oxytocin and placebo on neuropathic pain and mechanical hyperalgesia. The results showed that oxytocin reduced pain intensity more than placebo, but also increased the hyperalgesic area. Although limited by the small sample size, further research on spinal oxytocin in this population is warranted.

Systemic administration of a β2-adrenergic receptor agonist reduces mechanical allodynia and suppresses the immune response to surgery in a rat model of persistent post-incisional hypersensitivity

Vipin Arora, Carlos Eduardo Morado-Urbina, Young S. Gwak, Renee A. Parker, Carol A. Kittel, Enriqueta Munoz-Islas, Juan Miguel Jimenez-Andrade, E. Alfonso Romero-Sandoval, James C. Eisenach, Christopher M. Peters

Summary: Activation of Beta 2 adrenergic receptors has been shown to alleviate postoperative mechanical hypersensitivity, with more significant effects in depleted rats. Treatment with clenbuterol can reduce mechanical hypersensitivity, but has no significant impact on the speed of postoperative recovery. The study suggests that Beta 2 adrenergic receptors play a regulatory role in postoperative hypersensitivity in the nervous system.
