Summary: This study analyzed trends in patient demographic characteristics and comorbidities among individuals undergoing revision total joint arthroplasty (TJA) from 2008 to 2018. The results showed improvement in overall health status of patients during this time period, particularly in modifiable comorbidities.

Summary: The study identified modifiable risk factors for the development of GI complications following TJA, including older age, non-alcohol use, and several comorbidities. Patients with GI complications required longer hospital stays, greater post-acute care resources, and had a substantially higher risk of mortality.

Summary: Compared with general anesthesia, neuraxial anesthesia improves outcomes following outpatient hip and knee arthroplasty, including reduced pain, less opioid use, and lower incidence of postoperative nausea and vomiting.

Summary: This study investigated the trends and risk factors for delivery-related maternal deaths and severe maternal morbidity in the United States. The study found that maternal mortality rates decreased while severe maternal morbidity increased from 2008 to 2021. Age, race, and delivery method were associated with both outcomes.

Summary: Low surgeon and hospital volume are associated with higher complications during primary total hip arthroplasty and total knee arthroplasty, highlighting the importance of directing surgeries to high-volume centers and surgeons.

Summary: Patients undergoing total joint arthroplasty (TJA) experience sleep disturbances both before and after surgery, with improvements in sleep quality observed within the first 3 months postoperatively. The patterns of sleep changes differ based on preoperative sleep disturbance. Patients who undergo total hip arthroplasty are more likely to achieve clinically improved sleep by 3 months compared with those who undergo knee arthroplasty.

Summary: This study analyzed data from 2010 to 2020 to investigate the changes in inpatient and outpatient total knee arthroplasty (TKA) and found that the proportion of outpatient TKA increased significantly after 2018, with older, male, White, and higher-risk patients being more likely to undergo outpatient TKA.

Summary: This study developed and validated a practical and easy-to-use algorithm using machine learning to predict the risk of venous thromboembolism (VTE) and major bleeding (MBE) following total joint arthroplasty (TJA).

Summary: This study compared the incidence of major bleeding events (MBEs) in patients receiving aspirin as venous thromboembolism (VTE) prophylaxis vs other chemoprophylaxis following total joint arthroplasty (TJA). The results showed that patients who received aspirin had a lower risk of major bleeding. Factors such as age, body mass index, anesthesia score, and surgical factors were independently associated with an increased risk of MBE.

Summary: The study found that small non-teaching hospitals are more likely to refer failed primary TKA patients to medium to large teaching hospitals, especially in cases of infection. This referral pattern results in more medically complex patients being directed to medium to large teaching hospitals for revision TKA surgery.

Summary: This study investigates the trends in hospital revenue versus costs in primary total joint arthroplasty (TJA). The results show a gradual increase in direct costs and total costs, which may lead to a decrease in contribution margin if not controlled.

Summary: This study evaluated the effect of hospital size, teaching status, and indication for revision total hip arthroplasty (revTHA) on migration patterns in patients older than 65 years old. Results showed that migration rates were significantly higher in small non-teaching hospitals, particularly in cases of infection, periprosthetic fractures, instability, and mechanical complications. Over 80% of patients migrated to larger teaching hospitals.

Summary: During the COVID-19 pandemic, the number of elective inpatient total hip arthroplasty (THA) and total knee arthroplasty (TKA) cases in the United States significantly decreased, with a decrease of 46.5% to 47.7% in 2020 compared to the previous three years. The impact of the pandemic continued until the end of the year, with a steady decline in case volume.

Summary: This study analyzed the mark-up ratios for orthopaedic procedures (THA and TKA) performed on Medicare beneficiaries between 2013 and 2019. The findings showed that these procedures had significantly higher mark-up ratios compared to nonorthopaedic procedures. The excessive charges billed could potentially create financial burdens for patients and should be considered in future policy discussions to prevent price inflation.

Summary: The majority of canceled surgeries are due to medical reasons, with most patients eventually undergoing the procedure, but only a minority receiving intervention for their condition. Patients canceled for medical reasons tend to have higher Charlson Comorbidity Index, closer cancellation to the scheduled date, and are more likely to eventually undergo surgery.
