Summary: This study aimed to identify and describe the factors associated with patient-reported response after lumbar intra-articular facet joint injections or medial branch blocks. Facet joint osteoarthritis is a common cause of chronic low back pain. The success of these approaches is variable, prompting interest in identifying patient characteristics associated with response.

Summary: This study aims to quantify and compare needle angles during lumbar medial branch radiofrequency ablation, and the variability suggests that a standardized approach may not be optimal. Further research is needed to determine patient-specific needle angles.

Summary: Instrumented lumbar spinal fusion can lead to biomechanical changes and increased facet load bearing at adjacent spinal segments, causing facet-mediated pain. Medial branch nerve radiofrequency ablation is a treatment option, but the fusion approach must be considered to select the appropriate medial branch nerves.

Summary: This retrospective analysis of administrative data aimed to investigate the changing practice of repeated lumbar facet joint injections/medial branch blocks in England over a six-year period. The study found that the number of patients receiving repeated lumbar injections has decreased over time but has not been eliminated completely. There is a need for more efforts in educating patients and clinicians about alternative and more effective treatments.

Summary: This study aimed to investigate the ability of the Patient-Reported Outcomes Measurement Information System (PROMIS) to capture the therapeutic effect of first-time medial branch radiofrequency ablation (RFA). The study found that RFA for lumbar facet syndrome resulted in statistically significant improvements in PROMIS domains of Physical Function and Pain Interference, but no significant change in Depression.

Summary: This study found that ultrasound-guided lumbar medial branch blocks and facet joint injections are significantly associated with the risk of incorrect needle placement, especially when confirmed by fluoroscopy or CT. The study also suggested that technical limitations of ultrasound and individual patient factors could contribute to the risk of incorrect needle placement.

Summary: This study evaluated the effectiveness of ultrasound-guided hyaluronic acid (HA) epidural injections combined with neuromobilization in the conservative treatment of lumbar foraminal stenosis (LFS). The treatment showed improvement in the straight leg raise (SLR) test results, but did not have significant effects on pain intensity, disability level, or functional assessment. Some patients experienced symptoms flare-up shortly after the injection, but without neurological deficits. Overall, this method has the potential to enhance the neurodynamics of nerve roots in LFS and provide an alternative to steroid treatment.

Summary: Aortic stenosis is a common valvular disease requiring replacement, with transcatheter aortic valve replacement becoming a preferred treatment option. However, there is a risk of new conduction disorders necessitating permanent pacemaker implantation post-procedure, and current management strategies for these disturbances lack clarity.

Summary: This study determined the time criteria for postherpetic neuralgia (PHN) by analyzing the skin temperature of patients with herpes zoster. The transition of skin temperature from warm to cold occurs 12 weeks after the onset of herpes zoster, indicating the end of local inflammation. This finding provides a theoretical basis for diagnosing and treating PHN.

Summary: The study found that non-particulate steroids may form crystals when mixed with local anesthetics, possibly due to alkalinization of the steroids. Therefore, caution may be needed when mixing ropivacaine or levobupivacaine with betamethasone sodium phosphate during epidural steroid injections, and lidocaine or bupivacaine is recommended as a local anesthetic. Dexamethasone palmitate is a candidate for mixing, but further research on its safety and effectiveness is required.
