Summary: Surgery and anesthesia can cause stress to the brain, and monitoring with electroencephalography and cerebral oximetry can help assess anesthesia depth and cerebral metabolic activity. However, research results on the ability of these monitors to alleviate peri-operative neurocognitive complications have been contradictory. Understanding the changes in electroencephalography under anesthesia and those associated with aging is important for correctly interpreting anesthesia depth indices. Cerebral oximetry is useful for detecting cerebral desaturation and identifying higher-risk patients. An algorithm-based approach may be most effective in managing cerebral desaturation events.

Summary: This umbrella review systematically identified the peri-operative risk factors associated with post-operative cognitive dysfunction (POCD) through meta-analyses of observational studies. The review included 11 meta-analyses comprising 73 risk factors in a total population of 67,622 participants. Pre-operative risk factors were most commonly examined and there was limited evidence suggesting associations between two risk factors (pre-operative age and pre-operative diabetes) and POCD. Further large-scale studies across different surgery types are recommended due to the limited overall strength of the evidence.

The study found that the early use of CRRT had a low incidence and was associated with worse outcomes, including lower survival and higher postoperative creatinine levels. Factors such as preoperative severity and intraoperative hypotension episodes were found to be associated with the increased use of CRRT.

Summary: With low mortality rates for many cardiac surgical procedures, there has been a shift towards focusing on patient-centered outcomes such as recovery and quality of life. The Enhanced Recovery After Surgery (ERAS) cardiac society recently published guidelines to translate this philosophy for the benefit of patients undergoing cardiac surgery. Advances in areas such as mechanical circulation, diagnostics, and quality metrics are also contributing to improving care for patients undergoing cardiac surgery.
