We describe the case of a fit 17-year-old man who developed a severe allergic reaction to a low clinical dose of sugammadex (3.2 mg kg(-1), 200 mg intravenously), 1 min after its administration. This was manifest by an intense erythema over the anterior part of the thorax, severe lip and palpebral oedema and bilateral wheeze. On later investigation, the patient had a positive skin prick test to sugammadex (5-mm diameter response, with a negative saline control and positive histamine control of 5 mm) and no response to any other drug tested. Other diagnostic tests supported a diagnosis of allergic reaction to sugammadex.

Summary: This study aimed to characterize the onset of nut allergy in children in southern Europe. The results showed that the onset of nut allergy in children occurred around the age of 6, slightly later than reported in English-speaking countries. Walnut was found to be the main trigger, followed by peanut. 2S albumin storage proteins, particularly Jug r 1, were identified as the most relevant allergens.

Summary: The aim of this study was to identify serum miRNAs that can phenotype asthma patients. Next-generation sequencing and RT-qPCR validation revealed that hsa-miR-26a-1-3p and hsa-miR-376a-3p were associated with eosinophilic and noneosinophilic asthma. Furthermore, the expression of these miRNAs was correlated with the pathogenesis of asthma.

Summary: This real-world study found that biological treatments, including omalizumab, mepolizumab, reslizumab, and benralizumab, can improve olfaction in patients with severe asthma and associated CRSwNP. There were no significant differences in olfaction improvement between different biologic drugs, with factors such as atopy, steroid use, and polyp size influencing the improvement. The study also showed no differences in olfaction improvement between N-ERD and non-N-ERD groups.

Eosinophilia Induced by Blocking the IL-4/IL-13 Pathway: Potential Mechanisms and Clinical Outcomes

Summary: There are currently five biological drugs available for the treatment of severe asthma. These drugs primarily target specific pathways to alleviate inflammation. Dupilumab treatment may lead to eosinophilia in some cases, but it is usually temporary. Reports have shown that dupilumab may also be associated with eosinophil-related diseases. Therefore, the presence of eosinophilia should be carefully considered when choosing a treatment plan. In such cases, using anti-IL-5/IL-5R drugs may be more suitable. Additionally, close monitoring of blood eosinophil count and clinical progress is necessary when switching from anti-IL-5/IL-5R drugs to anti-IL-4/IL-13R drugs.

Summary: Real-life data show that over 50% of severe asthma patients treated with monoclonal antibodies do not achieve a complete response. The response can be measured qualitatively or quantitatively, and both methods are important for clinical decisions. Various factors contribute to suboptimal response, and a comprehensive evaluation is necessary to guide treatment decisions.

Summary: This study used unsupervised cluster analysis to identify four asthma phenotypes, including adult-onset, adolescent-onset, middle-age-onset, and elderly-onset. These phenotypes differ from each other in terms of lung function, age of onset, BMI, atopy, asthma severity, exacerbations, control, social class, smoking, and nasal polyps.

Summary: MicroRNAs (miRNAs) are increasingly being used as biomarkers for diagnosis and treatment of various diseases. This study analyzed miRNA expression profiles in serum and lung tissue from severe asthma patients with and without oral corticosteroids (OCS) treatment. MiRNA-Seq and RT-qPCR were used for evaluation and validation of miRNA levels. Altered expression of miRNAs in serum and lung tissue was observed, with hsa-miR-221-5p and hsa-miR-769-5p showing the best differentiation between OCS-treated and untreated individuals. These miRNAs were found to be associated with important signaling pathways for asthma and regulated the expression of genes involved in these pathways. The findings suggest that miRNA profiles can serve as biomarkers for OCS treatment in asthma patients.
