Summary: This study focused on 875 Black participants from the Coronary Artery Risk Development in Young Adults cohort to investigate the association between the V122I TTR genetic variant and cardiac structure and function in middle age individuals without heart failure. The findings revealed a significant link between the TTR V122I variant and worse left ventricular structure and cardiac mechanics at an average age of 54 years, suggesting that early screening with echocardiography in Black middle-aged adults carrying the TTR V122I variant can help guide the use of novel therapies and risk factor modification to prevent or delay heart failure.

Impaired left ventricular global longitudinal strain was associated with decreased left atrial and right ventricular function, highlighting the interdependence between atrioventricular and ventricular function, which could contribute to heart failure development.

Summary: Transthyretin cardiac amyloidosis (ATTR-CA) is a common cause of heart failure and mortality, with advances in diagnosis and treatment making it a condition that clinicians should consider daily. Amyloid fibril formation is caused by age-related failure of homoeostatic mechanisms or mutations. Large-scale studies show an increasing incidence of cardiac amyloidosis, with cardiac involvement being the leading cause of mortality. Early recognition is important for optimal treatment.

Summary: Cardiac amyloidosis (CA) affects all four heart chambers, and strain parameters of all chambers are altered in CA patients, particularly those with amyloid transthyretin (ATTR; n = 144, 34%) or amyloid light-chain (AL; n = 117, 28%) CA. Peak left atrial longitudinal strain (LA-PALS) is independently associated with the diagnosis of CA or ATTR-CA beyond standard echocardiographic variables and cardiac biomarkers. Patients with the most severe impairment of LA strain are most likely to have CA or ATTR-CA.

Summary: Subjective perception of symptom severity in patients with transthyretin-related cardiac amyloidosis (CA) is an important treatment target, as opposed to objective prognostic markers such as N-terminal pro b-type natriuretic peptide (NT-proBNP). However, anxiety and depression in CA patients may affect their perceptions of the disease, creating a potential gap between objective and subjective parameters. This study assesses the impact of such bias in consecutive CA patients.

Summary: This study suggests that left ventricular myocardial work may have prognostic value in cardiac amyloidosis patients by predicting major adverse cardiac events (MACE) and all-cause mortality. Combining LVMWI and the apical-to-basal segmental work ratio can independently predict all-cause mortality. LVGLS, on the other hand, does not predict all-cause mortality in this patient population.

Summary: This study aimed to investigate changes in echocardiographic parameters in ATTR-CM patients, differences in progression rate among different genotypes, and the predictive ability of echocardiographic parameter changes for prognosis. The results showed progressive worsening of echocardiographic parameters over time, with V122I ATTR-CM patients demonstrating the most rapid deterioration. Worsening of mitral and tricuspid regurgitation was associated with worse prognosis.

Summary: This study investigates the left atrial (LA) function and its prognostic implications in cardiac amyloidosis (CA) patients with preserved left ventricular ejection fraction (LVEF) and sinus rhythm. The findings indicate that assessing LA reservoir and contractile functions using 2D speckle tracking echocardiography can provide important clinical and prognostic insights. LA mechanics are found to be independent predictors of overall survival, and LA strain and strain rate improve the discrimination and goodness of fit of conventional prognostic models.

Summary: This study characterized left atrial pathology in explanted hearts with transthyretin amyloid cardiomyopathy (ATTR-CM); assessed left atrial mechanics using echocardiographic speckle tracking in a large cohort of ATTR-CM patients; and studied the association with mortality. The findings revealed extensive atrial infiltration of ATTR amyloid, loss of atrial function, and increased stiffness, which were strong independent predictors of mortality. A distinctive phenotype named atrial electromechanical dissociation (AEMD) was identified in patients in sinus rhythm with poor prognosis.

Summary: Cardiac involvement in amyloidosis is characterized by the deposition of misfolded proteins in the heart, which can be visualized with green birefringence under cross-polarized light. Recent data suggests that cardiac amyloidosis is underrecognized as a cause of common cardiac diseases. The prognosis for transthyretin amyloidosis is better than for amyloid light-chain amyloidosis, but not as favorable as for other etiologies of heart failure. Non-invasive imaging modalities, such as echocardiography and cardiac magnetic imaging, have advanced the diagnosis and management of cardiac amyloidosis.

Summary: There is a significant debate about the use of salt-restricted diet for managing heart failure. Existing dietary guidelines lack consistency and evidence. A study found that many patients do not comply with their doctors' recommended diet, and restrictive diets are associated with increased burden in heart failure patients. Therefore, an evidence-based approach is needed for developing and recommending heart failure-specific diets.
