Summary: A study found that livers with macrovesicular steatosis are less likely to be transplanted due to their higher risk of graft failure. To facilitate the use of these higher-risk organs, the SRTR now includes the variable of macrovesicular steatosis in their risk-adjustment models.

Summary: This study compares the outcomes of liver allografts preserved using portable normothermic machine perfusion (NMP) and ischemic cold storage (ICS) in liver transplantation procedures. It concludes that NMP can significantly reduce the incidence of early allograft dysfunction, reperfusion syndrome, and ischemic cholangiopathy after donation after circulatory death (DCD) liver transplantation.

Summary: This study evaluated the impact of acute kidney injury (AKI) on post-transplant clinical outcomes for deceased donor (DD) kidney transplantation (KT) using the Kidney Donor Profile Index (KDPI) system. The results showed that KT from DDs with AKI stage 3 had an adverse impact on the allograft outcome, especially in the high KDPI-KTR group. Therefore, careful management of deceased donors with high KDPI scores is important to prevent severe AKI prior to transplantation.

Summary: This study compares the outcomes of living donor liver transplant (LDLT) and deceased donor liver transplant (DDLT) in patients with primary biliary cholangitis (PBC). The results show that LDLT has superior patient survival compared to DDLT, and there is no significant difference in graft survival between the two methods. The study also identifies predictors of poor post-transplant outcomes in PBC patients.

Summary: Using unsupervised machine learning, the study identified three clinically distinct clusters of recipients in diabetic deceased donor kidney transplants with differing outcomes. The findings highlight opportunities to improve utilization of high KDPI kidneys from diabetic donors for recipients with survival-limiting comorbidities, as observed in Cluster 1.

Summary: Kidney transplantation from donors with acute kidney injury, especially in elderly donors, does not result in significantly worse outcomes compared to transplantation from donors without acute kidney injury. With careful selection, satisfactory outcomes can be achieved in kidney transplantation from elderly donors with acute kidney injury.

Summary: The shortage of pediatric multivisceral donors leads to long waiting times and high mortality rates for pediatric patients. The use of donors after cardiac death, particularly with normothermic regional perfusion, has increased for solid organ transplantation, except for the intestine. This case demonstrates the possibility of reducing waiting time for patients through multivisceral transplantation from donors after cardiac death.

Summary: A study on kidney transplant recipients found that deceased-donor AKI was independently associated with lower risk for BKV infection, contrary to the initial hypothesis. Data from the OPTN database also showed that donor AKI was linked to lower risk for graft failure attributed to BKV.

Summary: Acute kidney injury (AKI) severity is a strong risk factor for kidney nonprocurement in deceased organ donors. Efforts to address the organ shortage should focus on encouraging the procurement and utilization of kidneys from deceased donors with severe AKI, given the large prevalence of donor AKI and successful transplant outcomes with these kidneys.

Summary: The high demand for organs in kidney transplantation has led to the widespread use of machine perfusion technologies. This study provides a systematic review of the developments in this field over the past 10 years and compares different perfusion techniques. The results show that dynamic preservation strategies, such as hypothermic machine perfusion, have lower rates of delayed graft function and have the potential to improve outcomes in kidney transplantation.

Summary: A systematic review comparing adult living donor liver transplantation (LDLT) and deceased donor liver transplantation (DDLT) found that LDLT is associated with improved patient survival, reduced waiting time, and lower MELD score, despite an increased risk of biliary complications. Hepatic artery thrombosis rates did not differ between the two types of transplantation.

Summary: Liver transplantation can be done using deceased or living donor grafts. Deceased liver grafts, obtained from brain- or circulation-death patients, usually have some degree of damage. This study compared the metabolomics of deceased and living donor grafts. Deceased liver grafts had increased levels of certain lipid metabolites and amino acids compared to living donor grafts. The elevation of lipid metabolites can be used as an indicator of liver graft suffering.

Summary: This study aimed to evaluate the impact of the immune system after graft reperfusion during kidney transplantation in adults in order to identify predictive biomarkers. The study found that the percentage of T-lymphocytes in the liver perfusate was significantly associated with delayed functional recovery and secondary infection. However, there was no significant association between the proportions of NK cells and NK-T cells with clinical outcomes.

Summary: This article focuses on establishing reference case methods for expert elicitation to inform health care decision making. Methods choices for elicitation lack empirical evidence supporting recommendations, but reference case methods have been determined for health technology assessment. The reference methods prioritize experts with substantive knowledge, eliciting quantities directly observed by experts, and individual elicitation of beliefs. Further applied examples and experimental evidence comparing methods are recommended.

Summary: The study investigates the timing and intervention choice for chronic thoracic aortic aneurysms. It found that small aneurysms require close observation, while larger aneurysms require timely intervention. Endovascular stent grafting and open surgical replacement are successful treatments for carefully selected patients. The study also found that factors such as female gender, older age, and increasing diameter have a negative impact on patient survival and quality of life.

Both methods are applied to observational datasets and found to produce unbiased estimates, although estimates may be biased if the data are measured infrequently.

Summary: This study investigated the potential mediating factors, including lung function, pulmonary exacerbations, and nutritional status, through which cystic fibrosis-related diabetes (CFRD) affects the composite outcome of mortality or transplant in adult cystic fibrosis (CF) patients. The results showed that pulmonary exacerbations were the strongest mediator of the effect, attributing to a significant proportion of the total effect. Other factors may also contribute to the influence of CFRD on patient survival.

Summary: This study reveals time-varying differences in the hazard of all-cause mortality after liver transplantation between hepatocellular carcinoma (HCC) recipients and non-HCC recipients. It also provides evidence that some HCC patients have extra-hepatic spread at the time of transplantation, which has implications for post-transplant surveillance protocols.

Summary: This study analyzed the impact of modern glucose-monitoring strategies on glycemic and patient-related outcomes in individuals with type 2 diabetes (T2D) and recent myocardial infarction (MI). The results showed that compared with self-monitoring of blood glucose (SMBG), intermittently scanned continuous glucose monitoring (isCGM) marginally increased time in range and significantly reduced hypoglycemic exposure in T2D individuals with MI, while equally improving HbA(1c). These findings explain the cost effectiveness of isCGM.

Two rounds of online surveys resulted in 83 prioritized research questions related to critical lower-limb ischaemia, diabetic foot disease, amputation, wound healing, carotid plaque morphology, and service organization/delivery. These results are expected to drive the UK national vascular research agenda for the next 5-10 years.
