Summary: This study analyzed the hospitalization rates of patients with sarcoidosis in Ontario, Canada from 1996 to 2015. It found significant decreases in all-cause and sarcoidosis-related hospitalization rates, particularly among younger patients. Differences in hospitalization risk based on sex, socioeconomic factors, and geographic factors were observed in patients with sarcoidosis.

Summary: Sarcoidosis is a disease of unknown origin that can affect multiple organs. Cardiac sarcoidosis is particularly challenging due to its potential for serious cardiac complications and difficulty in diagnosis. This manuscript reviews the current understanding and guidelines for diagnosing and treating cardiac sarcoidosis, highlighting a case study of a young adult with no known risk factors or prior diagnosis of sarcoidosis presenting with disabling cardiac symptoms.

Summary: Sarcoidosis is a systemic autoimmune disease primarily affecting lymph nodes, lungs, skin, and eyes. It is relatively uncommon but not rare, with higher prevalence in northern Europe, the United States, and India. The disease predominantly affects women aged 30 to 50 and is closely linked to environmental factors, particularly occupational exposures. Diagnosis requires multidisciplinary clinical units to integrate clinical, analytical, radiological, and histopathological data. Treatment options include corticosteroids as first-line therapy, immunosuppressants as second-line therapy, and anti-TNF agents for severe and/or refractory cases.

Summary: The study observed an association between severity of pediatric atopic dermatitis (AD) and increased healthcare visits, as well as a significant linear trend between parental educational level and the fear of topical corticosteroids. Lower parental education level was linked to delayed treatment for AD flares.

Summary: The best quality data relate to atrioventricular nodal conduction and left ventricular function recovery. In both situations, therapy with corticosteroids and/or immunosuppressant therapy were sometimes associated with positive outcomes. The data quality is too limited to draw conclusions for ventricular arrhythmias and mortality.

Summary: Corticosteroids are widely used immunosuppressive agents for conditions like myasthenia gravis, but treatment with them may result in exacerbation of symptoms. The rate of exacerbation varies with different types of corticosteroids, with cortisone having the highest rate and methylprednisolone the lowest. Factors like age, symptom type, disease severity, and thymus-related factors may increase the risk of exacerbation.

Summary: Sarcoidosis is a complex granulomatous disease of unknown etiology. Diagnosis can be challenging due to the heterogeneous nature of the disease, often requiring a thorough and complex investigation. Starting unnecessary corticosteroid treatment can worsen the patient's prognosis, as many other granulomatous diseases can mimic sarcoidosis and lead to more difficult to treat side effects.

Summary: Individuals without healthcare utilization over a year have increased risks of all-cause and cardiovascular mortality, particularly more pronounced among males and uninsured individuals. Further investigation is needed to identify the underlying reasons for the elevated mortality risks due to lack of healthcare utilization.

Summary: This systematic review examined the hypothesis that gut microorganisms may contribute to the variability in the clinical response to classical immunosuppressant drugs (cIMDs) by affecting their pharmacokinetics, efficacy, or tolerability. The review found evidence supporting potential effects on cIMD pharmacokinetics, efficacy, or tolerability in studies of tacrolimus, cyclosporine, mycophenolic acid, and corticosteroids, while data were missing for everolimus and sirolimus. Furthermore, the review found that cIMDs have significant effects on the composition of intestinal microbiota, although no consistent pattern of microbiota modification was observed across studies.

Summary: This study reviewed a case series of cardiac sarcoidosis treated with TNF alpha antagonists, showing that adding TNF alpha antagonists to corticosteroid and immunosuppressive therapy can reduce relapse and major cardiac events without serious adverse events.

Summary: This study retrospectively examined the effectiveness of three treatment regimens for cardiac sarcoidosis and found that prednisone-tapering regimens containing infliximab were superior to those containing prednisone alone or prednisone plus methotrexate in terms of reducing prednisone dose and achieving the lowest effective dose.

Summary: This study aimed to describe the experience and perception of an owner who administered anti-inflammatory doses of oral prednisolone and prednisone to their dog. The results showed that by day 14, 90% of owners reported behavior changes in their dogs, with polyuria and polydipsia being the most commonly reported changes. Although most pet owners expressed satisfaction with steroid treatment, 70% would choose a more expensive treatment option with fewer side effects.

Summary: A prospective study on sarcoidosis clinic patients found that those who initiated or increased anti-inflammatory treatment had lower PGA scores. No significant difference in initial PGA score based on race, sex, or age was observed. The change in PGA was significantly lower in patients where treatment was increased.

Summary: Hypercalcemia can be caused by various factors, with hyperparathyroidism and malignancy being the most common. Sarcoidosis and vitamin D supplements can also lead to hypercalcemia, especially when combined. Monitoring serum levels of 25-OH Vit.D-3 and 1,25-(OH)(2) Vit.D-3 is crucial for identifying the underlying cause of hypercalcemia.

Summary: Advanced sarcoidosis is a form of sarcoidosis that poses a significant risk of organ function loss or death, with the lung being the most commonly affected organ. It can lead to complications such as pulmonary fibrosis and bronchiectasis. Although it affects a small proportion of patients, it is the leading cause of poor outcomes.
