The results showed that prolonged operative time is independently associated with severe complications. Summary: The aim of this study was to evaluate the management strategy and postoperative outcomes in patients undergoing surgery for presacral tumours. The study found that longer duration of surgery, poorer physical condition, higher location of the tumour, and combined resections were risk factors associated with postoperative complications. Meticulous planning and intensified perioperative care may improve outcomes in high-risk patients.

Individualized management is recommended based on patient's condition and anastomotic defect size, with non-operative treatment preferred for patients with proximal faecal diversion at the initial operation.

Patient-related risk factors were collected from national Danish registers, and prediction models were used to examine the impact on outcomes and variations between hospitals.
