Complement in Pregnancy: A Delicate Balance

The complement system is a key component of innate host defence that, under normal conditions, is responsible for the opsonization and destruction of potential pathogens. However, inappropriate or excessive activation of complement can have a detrimental effect on the host and has been implicated in the pathophysiology of numerous disease states. Recently, there has been increasing evidence for a role of the complement system and, in particular, the potent pro-inflammatory anaphylatoxin complement component 5a (C5a) in both normal and complicated pregnancy. The following review describes the results of in vitro, animal, and human clinical studies investigating the role of the complement system in healthy pregnancy, recurrent miscarriage, preterm birth, and preeclampsia.

Summary: This study aimed to examine the impact of implementing a service wide education program and antenatal care pregnancy weight gain chart combined with brief advice on women's knowledge of recommended gestational weight gain (GWG), the advice received and actual GWG. The results showed that service wide changes can improve the advice received by women and prevent excessive weight gain for women with normal weight.

TLQP-21 is a low potency partial C3aR activator on human primary macrophages

Summary: TLQP-21 is a neuropeptide derived from the VGF precursor protein and has pleiotropic roles in regulating metabolism, nociception, and microglial functions. TLQP-21 acts through several receptors, with C3aR being the most commonly reported. However, its activity in immune cells, which express C3aR, remains understudied. This study found that TLQP-21 induced ERK signaling in human and mouse macrophages, with mouse TLQP-21 being more potent than the human counterpart. These findings suggest a significant discrepancy in TLQP-21 C3aR activity between human and murine receptors in immune cells.

Summary: This study describes the characteristics and care provided to older people who died within 48 hours of emergency department (ED) presentation. Most patients arrived at the ED by ambulance and were triaged into the most urgent categories. Many of them had a history of independence but had a recent decline in mobility and activities of daily living. A significant proportion of patients had previous ED visits and hospital admissions prior to death.
