Summary: In this study, the researchers found that ICAM-1 is expressed on the sinus endothelia of lymph nodes, but not on the lymphatic vessels of peripheral tissues. Memory CD4(+) T cells migrated to lymph nodes from peripheral tissues more efficiently than from blood vessels. ICAM-1 deficiency significantly inhibited the migration of lymphocytes from peripheral tissues into lymph nodes, and the absence of ICAM-1 or LFA-1 reduced the number of memory T cells in lymph nodes.

Summary: Most patients with SLN+ melanoma no longer undergo a completion lymph node dissection (CLND) prior to adjuvant systemic therapy. The omission of immediate CLND in these patients may increase the risk of nodal relapse, but salvage surgery with therapeutic lymph node dissection (TLND) at relapse may be a viable option for these patients.

Summary: This study aimed to investigate the risk factors of non-sentinel lymph node metastasis in breast cancer patients with 1-2 sentinel lymph node macrometastases undergoing total mastectomy. The results showed that tumor size and ratio of sentinel lymph node macrometastases were independent risk factors for non-sentinel lymph node metastasis. The incidence of non-sentinel lymph node metastasis could be reduced to 17.1% for patients with tumor size ≤ 22 mm and ratio of sentinel lymph node macrometastases ≤ 0.33.

Summary: The aim of this study was to develop a nomogram based on elastography for the prediction of the risk of non-SLN metastasis in early-stage breast cancer patients with 1 or 2 positive SLNs in China. The nomogram showed good discrimination and performance in predicting NSLN metastasis, and could be considered as an ancillary tool to help selectively exempt patients from ALND.

Summary: This study evaluated the rate of non-sentinel lymph node (NSLN) positivity and factors influencing this in patients with a positive sentinel lymph node (SLN) following neoadjuvant chemotherapy (NAC). The results showed a high rate of nodal positivity on completion axillary lymph node dissection (cALND) in the setting of positive SLN after NAC, supporting the current standard of routine cALND. In patients with cN+ disease, NSLN positivity varied based on tumor biology, multicentricity/multifocality, number of positive SLNs, and SLN metastasis size.

Summary: Lymphatic metastasis is a major cause of cancer-related deaths, and the molecular basis of this process is not well understood. Cancer cells invade the lymphatic system and enter the sentinel lymph node, where they modify the microenvironment and suppress anti-tumor immunity. Developing strategies to reverse these changes in the sentinel lymph node may reduce colonization and distant metastasis. Alternatively, nanotechnology-based approaches for targeted drug delivery to the lymph node could enhance the efficacy of cancer treatment by killing cancer cells on site and promoting antigen presentation for immune activation.
