Posterior Vitreomacular Adhesion and Risk of Exudative Age-related Macular Degeneration: Paired Eye Study

PURPOSE: To evaluate posterior vitreomacular adhesion as a risk factor for choroidal neovascularization (CNV) in age-related macular degeneration (AMD). DESIGN: Retrospective, observational case series. METHODS. We retrospectively reviewed optical coherence tomography (OCT) and fluorescein angiography (FA) of 251 consecutive patients with unilateral exudative AMD. Fellow eyes had no sign of exudative AMD. Vitreomacular adhesion was defined when posterior hyaloid line attached to inner retinal surface was seen in OCT. We compared the incidence of posterior vitreomacular adhesion between the 2 eyes and the association between CNV location and vitreomacular adhesion. RESULTS: We found posterior vitreomacular adhesion in 56 patients (22.3%), and 3 cases in which it was present in both eyes. CNV was mostly present in eyes with vitreomacular adhesion (44/53, 83%), and rarely found in eyes without vitreomacular adhesion (6/53, 11.3%; P = .0007). The location of vitreomacular adhesion was always observed over the area of the CNV in exudative eyes (50/50). CONCLUSIONS: Posterior vitreomacular adhesion is associated with CNV in AMD. Chronic vitreomacular traction may be a risk factor for the development of exudative AMD. (Am J Ophthalmol 2009;147:621-626. (C) 2009 by Elsevier Inc. All rights reserved.)

Therapeutic effect of modified double-dose photodynamic therapy in circumscribed choroidal haemangioma

Evaluation of blood vessel network formation and visual field defect in optic disc melanocytoma

Summary: This study investigated the association between visual field defects and blood vessel network (BVN) formation in optic disc melanocytomas (ODMs) using optical coherence tomography angiography (OCTA). The results showed that the location of BVN absence within ODMs corresponded to the location of visual field defect and the location of fluorescein angiography (FA) hypofluorescence. Visual field defect was more severe in the incomplete BVN group than in the complete BVN group, and it was more significantly associated with the area of BVN absence than the tumor area.

Summary: This study examined the origins and natural course of focal scleral nodules using multimodal imaging. The majority of lesions were solitary and confined to the sclera. Most lesions showed outer retinal abnormalities and choroidal thinning or absence. The study found that these lesions are relatively stable over time.

Outcomes in PIC-Related CNV: Pooled Analysis of Individual Participant Data

Summary: The study systematically reviewed various treatment modalities for choroidal neovascularization (CNV) associated with punctate inner choroidopathy (PIC). Anti-VEGF agents were found to decrease the likelihood of moderate visual loss, although their effect was moderated by presenting visual acuity and patient age. Increasing the number of treatment modalities was associated with longer follow-up time, but did not improve vision.

Retinal Vascular Occlusion Risks in High Blood Pressure and the Benefits of Blood Pressure Control

Summary: This study aimed to evaluate the association between retinal vascular occlusion and stages of hypertension. The results showed that elevated BP, stage 1 hypertension, and stage 2 hypertension were all associated with a higher risk of retinal vascular occlusion compared to normal BP. Controlling hypertension appeared to reduce the risk, but the incidence rate was still significantly higher than that in individuals with normal BP.

PERIPHERAL EXUDATIVE HEMORRHAGIC CHORIORETINOPATHY IN ASIAN POPULATIONS

Summary: This study aimed to investigate the clinical characteristics and outcomes of peripheral exudative hemorrhagic chorioretinopathy (PEHCR) in Asian patients. The mean age of the patients was 70 years, with 75.9% of them being women. Vitreous hemorrhage occurred in 51.5% of eyes during follow-up, and it was associated with thicker baseline subfoveal choroid and male sex. The final visual acuity was worse than 20/200 in 29.2% of eyes, and factors predictive of poor final visual acuity included initial poor visual acuity, extensive peripheral retina involvement, subretinal hemorrhage extension to the macula, and absence of complete tumor regression.

Refractive Errors, Retinal Findings, and Genotype of Tuberous Sclerosis Complex: A Retrospective Cohort Study

Summary: The purpose of this study was to examine refractive errors, retinal manifestations, and genotype in tuberous sclerosis complex (TSC) patients in a Korean population. A total of 98 TSC patients were enrolled in a retrospective cohort study. The findings showed that TSC1 variant patients were more myopic, while TSC2 variant patients showed association with more extensive involvement of retinal astrocytic hamartoma.

Association between polycystic ovary syndrome and non-infectious uveitis

Summary: This study establishes a rapid and accurate diagnostic method for pathogen detection in infectious uveitis using nanopore metagenomic next-generation sequencing (NGS). Compared with conventional methods, nanopore sequencing can identify all causative pathogens with a shorter turnaround time. It is also capable of detecting pathogens even when the quantity or quality of DNA is suboptimal.

Genotypic Profile and Clinical Characteristics of CRX-Associated Retinopathy in Koreans

Summary: The study aimed to investigate the clinical characteristics of Korean patients with retinal dystrophy associated with pathogenic variants of the CRX gene. Eleven patients with CRX-RD were included in the study, and their clinical features and genotype-phenotype correlation were analyzed. The study found that pathogenic variants within the homeodomain of the CRX gene mainly resulted in cone-rod dystrophy or macular dystrophy with bull's eye maculopathy, while variants downstream of the homeodomain caused more diverse phenotypes, including cone-rod dystrophy, Leber congenital amaurosis, and retinitis pigmentosa. Further molecular research is needed to explore this correlation.

Best-corrected visual acuity deterioration was observed only in the generalized RP group. The study also identified several RHO gene mutations, including some that presented with both generalized and sector RP phenotypes.

Drusen and Other Retinal Findings in People With IgA Glomerulonephritis

Summary: Retinal drusen are uncommon in IgA nephropathy, but they resemble drusen found in age-related macular degeneration. The IgA-staining deposits in the drusen are likely related to systemic deposition of IgA. The nature of cystic spaces in the drusen is still unknown. Further research on the retinas of people with glomerulonephritis is recommended.

Validation of a Published Model to Reduce Burden of Retinopathy of Prematurity Screening

Summary: The study aimed to validate or refute the hypothesis that infants with birthweight over 750 g and gestational age over 27 weeks, who did not have retinopathy of prematurity (ROP) at 37 weeks, would not develop treatable ROP. The findings of the study supported the termination of examination at 37 weeks in infants meeting these criteria, as the value of continued screening was minimal.

Summary: This study aims to evaluate the research productivity of ophthalmology residency applicants and identify differences in productivity based on gender and race. The findings reveal that women have similar or higher research outputs compared to men, while self-identified underrepresented in medicine (URiM) students have lower research outputs in ophthalmology. Therefore, greater mentorship and support for URiM students are needed to promote the recruitment of diverse trainees in ophthalmology programs.
