Cerebroplacental ratio in predicting adverse perinatal outcome: a meta-analysis of individual participant data

Summary: This study found that in evaluating singleton pregnancies, the cerebroplacental ratio did not add predictive value for adverse perinatal outcomes beyond using umbilical artery pulsatility index alone.

Correlation of maternal body mass index with umbilical artery Doppler in pregnancies complicated by fetal growth restriction and associated outcomes

Summary: There is a positive correlation between increasing maternal BMI and abnormal UA Doppler findings in FGR. Maternal BMI may be considered as an additional risk factor when evaluating UA Doppler for placental insufficiency.

Severe early-onset fetal growth restriction: What do we tell the prospective parents?

Julia Unterscheider, Rocco Cuzzilla

Summary: Fetal growth restriction (FGR) is a common complication of pregnancy that can lead to higher perinatal mortality and adverse health outcomes for infants. Early detection and management of early-onset FGR through ultrasound and placental syndrome assessments are crucial to prevent very preterm birth and optimize outcomes.

Summary: This article reviews the literature on the diagnosis, antepartum surveillance, and time of delivery of fetuses suspected to be small for gestational age or growth restricted. It addresses the confusion caused by different recommendations from professional organizations and provides evidence-based guidelines for clinical management.

Renal transplant injury at caesarean delivery: A cautionary tale and a plan for the future

Ciara Anne McCormick, Paul Champion de Crespigny, Nancy Suh, Julia Unterscheider

Summary: Pregnancy following renal transplantation is becoming more common, with overall favorable outcomes. However, there are specific risks associated with the transplant, particularly a higher risk of caesarean delivery compared to the general obstetric population. Two recent cases of renal transplant injury during caesarean delivery highlight the potential risks of abdominal surgery. We propose a standardized approach to minimize harm and increase patient safety for transplant recipients undergoing caesarean delivery.

Neonatal outcomes following antenatal corticosteroid administration prior to elective caesarean delivery in women with pre-gestational diabetes: A retrospective cohort study

Iniyaval Thevathasan, Amalia Karahalios, Julia Unterscheider, Laura Leung, Sofia Walker, Joanne M. Said

Summary: This retrospective observational study aimed to evaluate the neonatal outcomes following antenatal corticosteroid (ACS) administration in women with pre-gestational diabetes mellitus (PGDM) who underwent elective caesarean section after 36 weeks gestation. Although there were no statistically significant effects observed, infants exposed to ACS before 38 weeks gestation were less likely to require respiratory support or neonatal nursery admission, while infants exposed after 37 weeks gestation were more likely to require parenteral treatment for neonatal hypoglycemia.

Medical termination of pregnancy for fetal anomaly at or beyond 20 weeks' gestation-What are the maternal risks?

Bridie Stewart, Stefan C. Kane, Julia Unterscheider

Summary: This study evaluates the common and severe maternal morbidities associated with medical termination of pregnancy (MTOP) for fetal anomalies ≥20 weeks' gestation. The study found that the most common complications were manual removal of placenta and postpartum hemorrhage.
