The quantitative lung index (QLI): a gestational age-independent sonographic predictor of fetal lung growth

OBJECTIVE: We sought to develop a gestational age-independent sonographic parameter to characterize lung growth. STUDY DESIGN: Reported descriptors of lung growth, including lung-to-head circumference (HC) ratio (LHR) and observed/expected LHR, were examined. A new index, the quantitative lung index (QLI) was derived using published data on HC and the area of the base of the right lung. RESULTS: Neither the LHR nor the observed/expected LHR proved to be gestational age independent. Right lung growth can be expressed using the following formula: QLI = lung area/(HC/10)(boolean AND)2. The 50th percentile of the QLI remained constant at approximately 1.0 for the gestational age between 16-32 weeks. A small lung (< 1st percentile) was defined as a QLI < 0.6. CONCLUSION: Fetal right lung growth can be adequately described using the QLI, independent of gestational age. Further studies are needed to assess the clinical accuracy of the QLI in characterizing fetal right lung growth.

Summary: This retrospective study investigated the antenatal course of 48 monochorionic diamniotic twins with selective intrauterine growth restriction (SIUGR) type III. The study found that approximately half of the patients progressed during the antenatal period, with the lack of progression being associated with a higher dual survivorship rate. Dual survivorship was significantly lower in the group that progressed compared to the group that did not progress.

Selective intrauterine growth restriction (SIUGR) type II: proposed subclassification to guide surgical management

Summary: This study aimed to characterize patients with twin-twin transfusion syndrome who would be excluded from laser surgery according to European criteria. The results showed that restricting the definition of twin-twin transfusion syndrome to a maximum vertical pocket for the recipient of >= 10 cm may potentially exclude 14.5% of patients from laser surgery, majority of whom had advanced stage of twin-twin transfusion syndrome.

Among 753 twins who underwent laser surgery, 6.9% experienced postoperative dual demise, with preterm birth being the most common cause. Nearly one-quarter of dual demise cases were deemed potentially preventable.

Summary: This study aimed to assess the difference in umbilical vein flow (UVF) between the recipient and donor twins with twin-twin transfusion syndrome (TTTS), and to evaluate the change in UVF following laser surgery. The results showed a significant preoperative difference in UVF between the recipient and donor twins, and a substantial increase in UVF of the donor twin postoperatively, confirming the effectiveness of laser surgery in improving perfusion.

Using Potentially Preventable Severe Maternal Morbidity to Monitor Hospital Performance

Summary: This study aims to adapt the Centers for Disease Control and Prevention (CDC) measure of severe maternal morbidity (SMM) for hospital performance measurement and evaluate it. The study found that the rate of potentially preventable SMM, as defined by pSMM, was less than half of the previously published rate of CDC SMM. Therefore, pSMM can be used for hospital performance comparison and quality improvement.
