Effect of Resident Gender and Surname Origin on Clinical Load: Observational Cohort Study in an Internal Medicine Continuity Clinic

Vanessa L. Kronzer, Emily L. Leasure, Andrew J. Halvorsen, Amy S. Oxentenko, Sara L. Bonnes

Summary: Studies show that patients may have gender or racial preferences for physicians. Female gender and European, non-Hispanic surname origin are associated with increased physician clinical load, even more than race. However, disparities in physician workload based on gender and name characteristics are addressable.

Optimizing the First Response to Sepsis: An Electronic Health Record-Based Markov Decision Process Model

Erik Rosenstrom, Sareh Meshkinfam, Julie Simmons Ivy, Shadi Hassani Goodarzi, Muge Capan, Jeanne Huddleston, Santiago Romero-Brufau

Summary: In this study, an optimal stopping model based on electronic health record data and clinical expertise was developed to identify the optimal first intervention action for sepsis treatment. The study found that implementing the optimal first treatment policy could reduce the average risk of death, time until treatment, and severity of treatment compared to current hospital treatment. The study also proposed an easily interpretable initial treatment heuristic based on patient's organ dysfunction, location, and septic shock status.
