Summary: The study aimed to investigate the association of 16 KIR complexes and HLA-C1 and C2 ligands with the genetic predisposition and development of Acute Lymphoblastic Leukemia (AML) in Saudi Arabian patients. The results showed significant differences in KIR genotype frequency between AML patients and healthy controls. KIR2DL1, KIR2DL5, and KIR2DS2 were significantly increased in patients, while KIR 2DL3, KIR2DS4, and KIR2DL2 were elevated in healthy controls.

Recurrent pregnancy loss (RPL), affecting 5% of pregnant women, with 40-60% cases unexplained, shares immunological similarities with allograft rejection. This review aims to explain how alterations in the HLA-G pathway disrupt immune balance and increase the risk of RPL.

The findings suggest that KIR2DS5, the C4T4 subset, and activating KIR genes are susceptible factors or positively associated with cervical cancer. Additionally, KIR2DL5B may have a protective association against cervical cancer, while KIR2DL1 and KIR2DL3 frequencies have important implications in healthy controls and HPV-infected women, respectively.

Summary: Pregnancy loss after IVF is as common as after spontaneous conception, and recurrent pregnancy loss may have an immunological background. This study investigated the effect of immunotherapy on women with RPL after IVF. The results showed that immunotherapy led to a higher live birth rate of 34%. This study indicates that immunotherapy may be a promising intervention for women with RPL after IVF.

Summary: Current evidence regarding the role of uterine natural killer (uNK) cells in recurrent implantation failure (RIF) and recurrent miscarriages (RM) is conflicting, with a definitive causative relationship still missing. Possible treatments for addressing uNK-related RIF and RM include glucocorticoids, intralipids, and intravenous immunoglobulin administration, but solid evidence on their efficiency and safety is needed before recommendation. Further studies are necessary to determine a causative relationship between uNK cells and RIF-RM pathologies before considering them as biomarkers or targets for therapeutic purposes.

Summary: The study aimed to evaluate the involvement of natural killer (NK) cells and regulatory T (Treg) cells in the beneficial effects of high dose intravenous immunoglobulin (IVIG) on recurrent pregnancy losses (RPL) of unexplained etiology. The results showed that IVIG increased Treg cell percentages and suppressed NK cell activity very early in pregnancy, and these changes were associated with subsequent live birth. Stimulation of Treg cells and suppression of NK cell activity may be a mechanism of the pharmacological effects of high dose IVIG.

Summary: In uncomplicated pregnancies, maternal-fetal HLA matching was not different than expected by chance, while in preeclamptic pregnancies, there was a higher degree of HLA-C matching than expected by chance. Preeclamptic pregnancies showed an overall tendency towards higher maternal-fetal HLA compatibility, suggesting a potential role of HLA mismatches in immune regulation leading to preeclampsia.

Summary: In recent years, advancements in reproductive technologies and genetic testing have led to improved embryo quality and higher pregnancy rates. However, some patients still face challenges with recurrent implantation failures and recurrent pregnancy losses, which could be attributed to immune dysregulations. Maintaining an anti-inflammatory environment before conception may yield superior results, and intravenous immunoglobulin G treatment has been shown to enhance reproductive outcomes in patients with immune dysregulations leading to RIFs and RPLs.
