Summary: This post-hoc analysis investigates the determinants, clinical implications, and outcome of persistent airflow limitation (PAL) in patients with asthma. The study reveals that PAL is not only present in severe asthma, but also in a considerable proportion of patients with milder disease. In patients with mild asthma, PAL is associated with eosinophilic inflammation and a higher risk of exacerbations, suggesting the need for increased treatment intensity.

Diet quality, food intake and incident adult-onset asthma: a Lifelines Cohort Study

Summary: This study aimed to explore the relation between diet quality and food intake with incident adult-onset asthma in normal weight and overweight adults. The results showed that diet quality was not associated with incident adult-onset asthma. Although the intake of several food groups differed between cases and controls, only a few remained associated with adult-onset asthma after adjusting for confounders.

Summary: Maintenance and reliever therapy (MART) with inhaled corticosteroid (ICS)/formoterol is as effective as fixed-dose fluticasone/salmeterol therapy in reducing exacerbations in moderate to severe patients with chronic obstructive pulmonary disease (COPD). Furthermore, MART requires a lower daily ICS dosage.

Summary: Smoking leads to a decrease in IL-33 expression at the gene and protein level, possibly due to a reduction in resting basal cells. These findings may explain why anti-IL-33 treatment is more effective in former smokers with COPD.

Summary: This study describes the pharmacokinetics of rifampicin (RIF) and isoniazid (INH) in patients with drug-susceptible tuberculosis in a real-world setting. Summary: This study compared the efficacy and safety of adding a long-acting muscarinic antagonist (LAMA) to medium-dose ICS/LABA in patients at GINA 2022 step 4. The results showed that medium-dose MF/IND/GLY improved lung function and reduced asthma exacerbations at Week 26, compared to high-dose MF/IND and high-dose FLU/SAL. Furthermore, medium-dose MF/IND/GLY also showed additional improvements in other lung functions.

Clinical practice of non-invasive ventilation for acute exacerbations of chronic obstructive pulmonary disease

Summary: This study evaluated guideline adherence to non-invasive ventilation (NIV) initiation for acute exacerbations of chronic obstructive pulmonary disease (COPD) and its impact on mortality. The overall guideline adherence rate was 82%, and better survival was observed in patients who received NIV when indicated. Therefore, increasing awareness and reducing reluctance in utilizing NIV through healthcare professional training can further enhance NIV treatment in clinical practice and improve patient outcomes.
