Risk Factors Promoting Hypertensive Crises: Evidence From a Longitudinal Study

BACKGROUND Current knowledge about risk factors promoting hypertensive crisis originates from retrospective data. Therefore, potential risk factors of hypertensive crisis were assessed in a prospective longitudinal study. METHODS Eighty-nine patients of the medical outpatient unit at the University Hospital of Bern (Bern, Switzerland) with previously diagnosed hypertension participated in this study. At baseline, 33 potential risk factors were assessed. All patients were followed-up for the outcome of hypertensive crisis. Cox regression models were used to detect relationships between risk factors and hypertensive crisis (defined as acute rise of systolic blood pressure (BP) >= 200 mm Hg and/or diastolic BP >= 120 mm Hg). RESULTS The mean duration of follow-up was 1.6 +/- 0.3 years (range 1.0-2.4 years). Four patients (4.5%) were lost to follow-up. Thirteen patients (15.3%) experienced hypertensive crisis during follow-up. Several potential risk factors were significantly associated with hypertensive crisis: female sex, higher grades of obesity, the presence of a hypertensive or coronary heart disease, the presence of a somatoform disorder, a higher number of antihypertensive drugs, and nonadherence to medication. As measured by the hazard ratio, nonadherence was the most important factor associated with hypertensive crisis (hazard ratio 5.88, 95% confidence interval 1.59-21.77, P < 0.01). CONCLUSIONS This study identified several potential risk factors of hypertensive crisis. Results of this study are consistent with the hypothesis that improvement of medical adherence in antihypertensive therapy would help to prevent hypertensive crises. However, larger studies are needed to assess potential confounding, other risk factors and the possibility of interaction between predictors.

The study found that the amplitude and duration of HBmax were higher and longer, respectively, in patients with AVNRT compared to those with atrioventricular reentrant tachycardia (AVRT).

Summary: This study investigated the exercise performance and ventricular function of ARVC patients during follow-up, taking into account their adherence to exercise restriction recommendations. The results showed that if intensive sports activities are not discontinued, exercise capacity and left ventricular function of athletes with ARVC deteriorates over time. Therefore, it is essential for all ARVC patients to strictly adhere to the recommendation to cease intense sports activity in order to halt disease progression.

A Novel Heterozygous Desmoplakin Variant Causes Cardiocutaneous Syndrome with Arrhythmogenic Cardiomyopathy and Palmoplantar Keratosis

Summary: In this case report, a 58-year-old male patient with cardiomyopathy and palmoplantar keratosis is described. Genetic testing revealed a novel variant in the desmoplakin gene, suggesting the presence of Cardiocutaneous syndrome. Repeated genetic testing is clinically significant, especially in patients with a high probability of a specific inherited cardiac disease, to improve diagnostic accuracy.

3D Atrial Strain for Predicting Recurrence of Atrial Fibrillation after Pulmonary Vein Isolation

Summary: This study investigated the association of 2D and 3D left atrial strain with low-voltage area in relation to the recurrence of atrial fibrillation after pulmonary vein isolation. The results showed that 3D left atrial reservoir strain and pump strain were lower in patients with recurrent AF. Furthermore, the association of 3D left atrial strain with recurrence of AF was independent of other clinical and echocardiographic parameters, making it a potential predictor for outcomes in patients undergoing PVI.

Circulating Biomarkers of Fibrosis Formation in Patients with Arrhythmogenic Cardiomyopathy

Summary: Arrhythmogenic cardiomyopathy (ACM) is characterized by a fibro-(fatty) replacement of the myocardium. However, current cardiac imaging modalities have limitations in detecting fibrosis patterns. In this study, serum samples were collected from 45 ACM patients and the levels of PICP and ICTP were measured. The results showed that diagnosed patients had higher levels of PICP and PICP/ICTP compared to preclinical variant carriers, indicating an increased collagen deposition. Furthermore, negative correlations were found between PICP/ICTP ratio and right ventricular ejection fractions, as well as significant correlations with left ventricular function. These findings suggest that PICP/ICTP levels may serve as potential biomarkers for ACM.
