Summary: Limited information is available on the effect of antiretroviral treatment (ART) on vascular function in South Africans of African descent living with human immunodeficiency virus (HIV). This study found that ART was associated with cardiovascular risk factors, particularly dyslipidaemia, in hypertensive South Africans of African ancestry with HIV. However, ART was not associated with endothelial dysfunction, and flow-mediated dilatation was positively associated with CD4 count in HIV-positive participants on ART.

Summary: The study found an exaggerated vasodilating microvascular response in cirrhotic patients with septic shock. This may explain vasopressor resistance and opens the door for future therapeutic trials targeting the nitric oxide pathway specifically in this population.

Summary: The study found that higher inflammation scores were associated with smoking, older age, higher BMI, systolic blood pressure, lipids, and glucose levels, as well as greater odds of hypertension and diabetes. Additionally, higher baseline scores were linked to increased risk of new-onset cardiovascular disease events and all-cause mortality.

Summary: Endothelial cells play a crucial role in cardiovascular health, and dysfunction can be caused by cardiovascular risk factors but restored once stimuli are removed. There are various methods to measure endothelial function, with each having its pros and cons. It is recommended to adopt a consensus methodology like flow-mediated dilatation and establish reference values for different populations. Future studies should explore the integration of endothelial function measurements with patient-specific epigenetic data and other biomarkers.

Summary: The study showed that chronic kidney disease (CKD) patients can benefit from nutrition education (NE) and lifestyle adjustments in improving kidney function and cardiovascular risk factors, particularly for patients who smoke or have inactive physical activity. Nutrition education may play a positive role in enhancing kidney function and improving cardiovascular risk factors.

Summary: This study found that the presence of cardiovascular risk factors, especially hypertension, in patients with neovascular age-related macular degeneration negatively impacts visual acuity outcomes following anti-VEGF therapy. Patients with higher total cholesterol levels showed better visual improvement, but this did not significantly affect the percentage of responders.

Summary: This study provides a comprehensive analysis of the determinants of hypertension (HTN) in hemophiliacs. It discusses the impact of factors specific to hemophilia, such as type, severity, and the presence of inhibitors. Additionally, it addresses the complex mechanisms of kidney dysfunction, endothelial dysfunction, and the association between HTN and hemophilic arthropathy. The clustering of cardiovascular risk factors in hemophiliacs exacerbates the negative vascular effects of HTN and increases the risk of coronary and cerebrovascular events.

Summary: Endothelial dysfunction is a precursor to atherosclerosis and is associated with the coexistence of CVD and CKD. This study found that retinal arteriolar endothelial dysfunction is present in CVD patients with early-stage CKD and serves as an indicator of long-term CKD progression in those with normal renal function.

Summary: The endothelium plays a crucial role in maintaining and regulating vascular integrity. In patients with chronic kidney disease, endothelial cell dysfunction contributes to increased cardiovascular complications due to the proinflammatory, prothrombotic, and uremic environment caused by declining kidney function. This review discusses the functioning of endothelial cells in healthy conditions, the phenotypic changes in chronic kidney disease patients, and the relationship between endothelial cell dysfunction and cardiovascular risk in chronic kidney disease. Additionally, it considers potential pharmacological interventions to improve endothelial health.

Summary: This study investigated macro- and microvascular function parameters and their relationship with cardiovascular risk markers in patients with untreated moderate to severe obstructive sleep apnoea. The results showed that individuals with untreated moderate to severe OSA exhibited signs of increased arterial stiffness and retinal microvascular dysfunction, which could be early indicators for future vascular complications.

Summary: This study systematically evaluated the relationship between cardiovascular disease risk factors and patients with systemic lupus erythematosus (SLE), and found that older age, male sex, hypertension, dyslipidemia, smoking, and a family history of cardiovascular disease were traditional risk factors, while long-term disease duration, lupus nephritis, neurological disorders, high disease activity, organ damage, and the use of glucocorticoids, azathioprine, and antiphospholipid antibodies were SLE-specific risk factors. However, the study quality of all included systematic reviews was critically low.

Summary: The aim of this study was to describe the incidence of recurrent thrombosis (RT) in patients with non-cirrhotic splanchnic vein thrombosis (NC-SVT) and identify its risk factors. The study found that the risk of RT in NC-SVT patients without an indication for long-term anticoagulation is not low. Furthermore, factor VIII level may be a key indicator for predicting RT.

Summary: This study investigates the efficacy of pre-emptive TIPS (pTIPS) in patients with bleeding from gastric fundal varices. The results show that pTIPS can significantly improve the rebleeding-free survival in these patients compared to standard therapy. Therefore, the use of pTIPS should be considered in patients with Child-Pugh B or C scores bleeding from gastric fundal varices.

Summary: Cirrhosis has two stages: compensated and decompensated. Decompensated stage is characterized by the development or presence of ascites, variceal hemorrhage, and hepatic encephalopathy. Treatment options vary depending on the specific complications, such as using nonselective b-blockers to prevent decompensation in patients with clinically significant portal hypertension or using trans-jugular intrahepatic portosystemic shunt (TIPS) for high-risk variceal hemorrhage. Other treatments like retrograde transvenous obliteration and variceal cyanoacrylate injection are emerging alternatives for specific conditions.

Summary: Liver sinusoidal endothelial cells (LSEC) are key elements in regulating the liver response to injury and regeneration. Endothelial autophagy plays a crucial role in protecting cells from injury-induced oxidative stress and fibrosis.
