Summary: This study aims to characterize gastroesophageal reflux disease (GERD) following laparoscopic Heller myotomy with Toupet fundoplication (LHM+T) in treated achalasia patients. The incidence of postoperative GERD burden is relatively low, but accurate diagnosis and differentiation from poor esophageal clearance in treated patients are important. Symptom correlation to abnormal pH study is unreliable, necessitating objective postoperative testing and manual review of abnormal pH studies.

Summary: The aim of this study was to assess the safety and effectiveness of resection procedures with different circumferences in patients with treatment-refractory GERD. The results showed that ARMS is an effective treatment for treatment-refractory GERD, and the recommended resection procedure is 2/3 circumference, 1.5 cm wide mucosal resection of the gastric cardia.

Summary: Fexuprazan 40 mg is non-inferior to esomeprazole 40 mg in healing erosive esophagitis at week 8. The study suggests that Fexuprazan may be a promising alternative treatment option to PPIs for patients with erosive esophagitis.

Summary: This study aimed to analyze and compare quality of life outcomes following LARS among patients with and without ineffective esophageal motility. The study found that patients with ineffective esophageal motility may experience lower improvements in dysphagia post-surgery, but still derive benefits in perioperative and quality of life outcomes.

Summary: The study found that elderly patients undergoing laparoscopic antireflux surgery are more likely to experience intraoperative complications, require reoperations, and have longer operative and hospital stay times. However, postoperative quality of life and complication rates are similar among all age groups.

Summary: The study demonstrates the effectiveness of LARS in treating therapy-resistant GERD in children. Five years post-surgery, reflux symptoms are still improved, but a decline in symptom-free patients over time was observed. The initial increase in HRQoL shortly after LARS appears to decline over time.

Summary: Abnormal DeMeester score on pH monitoring is a well-established predictor of favorable outcome after antireflux surgery. Symptom association probability (SAP) is often used as an adjunct to DeMeester score during preoperative assessment. However, the utility of SAP in predicting outcomes after fundoplication is limited.

Summary: This study aimed to determine the impact of previous abdominal surgery (PAS) on the short- and long-term outcome of fundoplication (FP) for refractory gastroesophageal reflux disease (GERD) in patients aged < 23 years. The results showed that patients with PAS required longer operative times and hospital stays, but had comparable rates of surgery-related short- and long-term complications to those without PAS.

Summary: In this study, the long-term outcomes of laparoscopic and open fundoplication in children were compared. The recurrence rate was found to be higher and recurrence occurred earlier in laparoscopic surgery compared to open surgery. However, both surgical approaches resulted in high patient and parental satisfaction.

Summary: Low anterior resection syndrome has a significant impact on the quality of life in rectal cancer survivors. This study found that there is no difference in the functional outcome between patients undergoing laparoscopic versus open resection for rectal adenocarcinoma, suggesting that the choice of surgical approach does not influence the likelihood or severity of low anterior resection syndrome.

Summary: This qualitative study investigated the decision-making process of surgeons regarding protective ileostomy creation after rectal cancer surgery. The results showed that 76% of surgeons supported the creation of ileostomy. When making the decision, 88% of surgeons considered risk factors. Emotions influenced the decision-making process in 22% of cases, and personal experiences influenced it in 49% of cases. The most frequently considered risk factors were the distance between the anus and the tumor (96%), neoadjuvant chemoradiotherapy (88%), positive intraoperative leak test (65%), blood loss (37%), and immunosuppression therapy (35%).

Summary: This study compared the outcomes of Self-Expandable Metal Stents (SEMS) and Endoscopic Vacuum Therapy (EVT) in the treatment of anastomotic leaks after esophagectomy/gastrectomy, with a focus on oncologic surgery. The results showed that EVT was more effective and had fewer complications compared to SEMS. However, in the oncologic surgery subgroup, there was no significant difference in the success rate between the two treatments.

Summary: Esophageal adenocarcinoma (EAC) is a severe malignancy with increasing incidence, poorly understood pathogenesis, and low survival rates. TP53 gene alterations, especially missense mutations, negatively affect cancer-specific survival in EAC. HNF1alpha gene disruption is associated with other gene alterations and is identified as a new EAC-mutated gene.

Summary: This study reveals the relationship between KRAS mutation and let-7a downregulation in cigarette smoke-induced lung carcinogenesis. Long-term exposure to cigarette smoke leads to a significant decrease in let-7a expression and an increase in KRAS mutation, resulting in a higher incidence of lung cancer.

Summary: The study investigated the use of the diatom Phaeodactylum tricornutum Bohlin (Phaeodactylaceae) as a functional ingredient for nutraceutical and cosmeceutical applications. It was found that the extract from this diatom, which contains high levels of neutral lipids and xanthophylls, exhibited excellent antioxidant activity and photostability against UVA and UVB rays. In addition, it was also shown to effectively reduce oxidative and inflammatory stress markers induced by solar radiation, making it a potentially safe and effective ingredient in broad-spectrum sunscreens.

Summary: This study investigated the impact of different fixation times/protocols on the quality of MMR protein immunohistochemistry (IHC) in colorectal cancer. The results showed that both hypo-fixation and hyper-fixation of formalin can cause problems, and 24-hour formalin fixation and cold formalin fixation are recommended for successful evaluation of MMR protein IHC.
