Summary: Surgery is the standard treatment for most basal cell carcinomas (BCC), but in some cases, radiotherapy, ablative and topical treatments may also be valuable options. Locally advanced BCCs (laBCC) and metastatic BCC, also referred to as 'difficult-to-treat' BCC, pose a challenge for current treatment approaches. The development of new selective therapies targeting the Hedgehog (HH) pathway, such as sonidegib, has shown promising results in managing difficult-to-treat BCC.

Hedgehog inhibitors Sonidegib and Vismodegib are first-line treatments for advanced BCC, but long-term use of hedgehog inhibitors is often challenging due to tolerability issues. However, various strategies for adverse effect management, such as dose interruptions, on-label alternate-day dosing, and supportive medications, are available.

Summary: Previous studies have shown that acne-prone skin exhibits microscopic abnormalities such as microcomedos and infundibular hyperkeratosis. In this study, treatment with probiotics and a topical product led to a reduction in both clinical and microscopic acne features. Reflectance confocal microscopy and dynamic optical coherence tomography were effective tools for monitoring subclinical changes in acne-prone skin.
