Summary: This study found that data on long-term cardiac and cerebrovascular adverse events in patients with hypertrophic cardiomyopathy (HCM) and apical aneurysm (AAn) are scarce, and specific treatment strategies that include the use of anticoagulants have not yet been established. The results showed that AAn is independently associated with an increased risk of HCM-related adverse events, especially cerebral infarction, and there is a significant relationship between aneurysm size and adverse events.

Summary: This study explored the long-term changes in the hemostatic system in patients with Takotsubo cardiomyopathy (TTS). The results showed that TTS patients exhibit persistent abnormalities in coagulation function compared to control subjects and patients with coronary artery disease (CAD), which may be related to TTS recurrence and adverse outcomes. Additionally, TTS patients have higher residual platelet activity compared to CAD patients, suggesting the need for investigation on TTS-specific antiplatelet therapy to prevent adverse outcomes.

Summary: This study compared the transplant-free survival, incidence of sudden cardiac death (SCD), and use of implantable cardioverter-defibrillators (ICDs) between RAS-HCM and P-HCM patients. The results showed that RAS-HCM patients were diagnosed at a younger age compared to P-HCM patients, had a higher incidence of nonarrhythmic deaths or transplants, but a similar incidence of SCD. However, the use of ICDs was lower in RAS-HCM patients compared to P-HCM patients.

Summary: This study aimed to determine the occurrence of cardioembolic ischemic events (CIEs) in patients with arrhythmogenic cardiomyopathy (ACM) and identify predictors of CIEs. The results showed that 10% of the ACM patients experienced CIEs, and factors such as female sex, T-wave inversion, RVOT dilatation, and lower LVEF were associated with CIEs. Among patients with atrial arrhythmias, 21.7% experienced CIEs. CHA2DS2-VASc score was not useful in predicting CIE risk in these patients. Rating: 8/10.

Summary: The study assessed the utility of a multidisciplinary approach in optimizing diagnosis of sudden cardiac arrest (SCA) etiology during follow-up. Results showed that systematic comprehensive testing combined with multidisciplinary expert team review improved the yield and time to diagnosis in SCA survivors without structural heart disease, compared with previous studies. The approach has positive implications in the management of SCA survivors and their families.

The results showed differences in rates of cardiac specialist follow-up and AVR and risk of mortality between primary care provider- versus cardiologist-managed patients with sAS. In addition, a lower likelihood of receiving follow-up and AVR was independently associated with higher mortality.

The intervention included redesigned workflows and a novel CDS tool for the coordination and delivery of guideline recommendations by primary care teams in a rural clinic. The pilot trial demonstrated feasibility and potential for successful implementation of the sociotechnical intervention.

Summary: This study aimed to assess lifetime morbidity and long-term survival in patients with bicuspid aortic valve (BAV) in the community. The results showed that BAV patients have a high lifetime morbidity burden, with a high incidence of complications such as aortic stenosis or regurgitation, aortic surgery, aortic aneurysm, infective endocarditis, and aortic dissection. Compared to patients with typical valvulo-aortopathy, patients with complex valvulo-aortopathy have lower survival rates.

Summary: The aim of this study was to investigate the incidence, risk factors, and clinical significance of atrial functional mitral regurgitation (AFMR) in patients with atrial fibrillation (AF) or sinus rhythm (SR). The results showed that the incidence rate of >mild AFMR was 2.6 per 100 person-year in new-onset AF and 0.7 per 100 person-year in SR. Age, sex, left atrial volume index (LAVI), and diastolic dysfunction (DD) were independent risk factors for incident AFMR. AFMR was associated with increased risk of all-cause death.

Summary: This study aimed to evaluate the predictive value of the GRACE risk score in acute pulmonary embolism (PE) patients for 30-day mortality and the need for thrombolytic treatment. The study found that the GRACE risk score, along with the PESI score, were independent risk factors associated with 30-day mortality and the need for thrombolytic treatment. The GRACE risk score showed high sensitivity and specificity in predicting mortality, and its combination with the PESI score helped define high-risk PE patients and predict poor prognosis.

The algorithm had a higher accuracy than the current guidelines' criteria in detecting severe TR by MRI.

Cardiac catheterization is uncommon in patients with this particular abnormality, highlighting the importance of customizing techniques to engage coronary arteries and optimize guide support for percutaneous coronary intervention if needed.

Summary: This study evaluated the angiogenic capacity of adipose tissue in patients undergoing cardiac surgeries and found that patients with coronary artery disease (CAD) had reduced angiogenic capacity in their pericardial adipose tissue. Gene expression analysis revealed upregulated expression of the anti-angiogenic gene TSP-1 in CAD patients, with no significant differences in other angiogenic factors. Additionally, inhibiting TSP-1 expression significantly improved angiogenic deficiency in CAD patients.

Summary: The current guidelines recommend prophylactic implantable cardioverter-defibrillator (ICD) for patients with symptomatic heart failure (HF) and low left ventricular ejection fraction. However, not all patients will benefit from ICD treatment. This study evaluated the feasibility of using the MADIT-II-based Risk Stratification Score (MRSS) to predict the survival benefit of prophylactic ICDs in patients with HF. The results showed that different risk subgroups had varying levels of ICD survival benefit.

Summary: Using latent class analysis (LCA), the study identified clinical, demographic, and social subphenotypes in ASCVD population and assessed the risk of all-cause and cardiovascular mortality across different socioclinical classes. The results showed that the younger, female, non-Hispanic Black or Hispanic individuals with a high burden of co-morbidities and unfavorable social determinants of health had the highest risk of mortality in the identified latent classes.

Summary: This study aimed to examine the relation between reverse septal curvature (RSC) and nonsustained ventricular tachycardia (NSVT) in patients with hypertrophic cardiomyopathy (HCM). The study found that patients with RSC were younger and had a higher incidence of NSVT, and RSC was identified as a strong independent risk factor for NSVT.
