Summary: In Western Denmark, the implementation of primary percutaneous coronary intervention (pPCI) for ST-segment elevation myocardial infarction (STEMI) has led to a significant reduction in 1-year mortality, with the greatest decline occurring within the first 30 days after pPCI. This highlights the importance of optimizing early management for improving overall survival in STEMI patients.

Summary: This study evaluated the role of ST-segment resolution (STR) and Thrombolysis in Myocardial Infarction (TIMI) flow in reperfusion evaluation after primary percutaneous coronary intervention (PPCI) for ST-segment-elevation myocardial infarction. The results showed that STR =50% and complete STR were significantly associated with lower 2-year mortality compared to STR <50%. Additionally, the combination of STR and TIMI flow provided incremental prognostic information beyond either measure alone.

Summary: The study found that women are less likely to receive primary percutaneous coronary intervention (pPCI) than men. However, the reduction in in-hospital mortality with pPCI is comparable between men and women. Despite this, the rates of pPCI remain lower in women in contemporary US practice.

Summary: This study compared the clinical outcomes of early versus late delayed PCI in elderly patients with STEMI. The results showed that early delayed PCI (12-48 h from symptom onset) is not associated with better long-term clinical outcomes compared with late delayed PCI (48 h-28 days).

Summary: This study aimed to investigate the dynamical change of PTX3 after pPCI in STEMI patients and its prognostic value. The results showed that the change in PTX3 from admission to post-pPCI 24-hour had a potential value in independently predicting the incidence of 1-year MACCEs in STEMI patients, especially for coronary revascularization.

Summary: This study found that the stress hyperglycemia ratio (SHR) is associated with the risk of pulmonary infection during hospitalization and in-hospital major adverse cardiovascular events (MACEs) for patients with ST-segment elevation myocardial infarction (STEMI) undergoing percutaneous coronary intervention (PCI). Higher SHR values were associated with higher rates of pulmonary infection and in-hospital MACEs. Multivariate logistic regression analysis confirmed the significant relationship between SHR and these outcomes after adjusting for potential confounding factors.

Summary: The study investigated the effectiveness and safety of half-dose tenecteplase in older patients with ST-segment-elevation myocardial infarction (STEMI) who were unable to undergo timely primary percutaneous coronary intervention (PCI). The results showed that the use of half-dose tenecteplase in a pharmaco-invasive strategy was associated with comparable electrocardiographic changes to primary PCI. The clinical efficacy and angiographic outcomes were similar in both treatment groups, but the risk of intracranial hemorrhage was higher in the half-dose tenecteplase group.

Summary: The study investigated the selection of critical boundary functions for testing hypotheses of two time-to-event outcomes in group-sequential clinical trials. It considered unequal effect sizes of endpoints and different evaluation periods for the endpoints. The effects of hazard ratios magnitudes and correlation between endpoints on statistical powers were assessed to provide guidance for consideration.

Summary: This study investigated the factors related to early initiation of rivaroxaban and the differences in clinical outcomes of stroke patients with nonvalvular atrial fibrillation (NVAF) who underwent reperfusion therapy. The results showed that infarct size and successful reperfusion were associated with the timing of starting rivaroxaban. There were no significant differences in the rates of ischemic events and major bleeding between patients who started rivaroxaban < 72 hours and ≥72 hours after the index stroke.

Summary: This study aimed to evaluate the predictive value of the GRACE risk score in acute pulmonary embolism (PE) patients for 30-day mortality and the need for thrombolytic treatment. The study found that the GRACE risk score, along with the PESI score, were independent risk factors associated with 30-day mortality and the need for thrombolytic treatment. The GRACE risk score showed high sensitivity and specificity in predicting mortality, and its combination with the PESI score helped define high-risk PE patients and predict poor prognosis.

Summary: Transthoracic echocardiography (TTE) is commonly used to evaluate isolated tricuspid regurgitation (TR), but there is limited research comparing its quantification with magnetic resonance imaging (MRI). In this study, a novel TTE algorithm was developed to identify severe TR based on TTE parameters. The algorithm had a higher accuracy than the current guidelines' criteria in detecting severe TR by MRI.

Summary: This article presents a case of a patient with known situs inversus who underwent cardiac catheterization and was found to have chronic total occlusion in the right coronary artery. Situs inversus is a rare congenital abnormality characterized by the inverted position of chest and abdominal organs. Cardiac catheterization is uncommon in patients with this particular abnormality, highlighting the importance of customizing techniques to engage coronary arteries and optimize guide support for percutaneous coronary intervention if needed.

Summary: This study evaluated the angiogenic capacity of adipose tissue in patients undergoing cardiac surgeries and found that patients with coronary artery disease (CAD) had reduced angiogenic capacity in their pericardial adipose tissue. Gene expression analysis revealed upregulated expression of the anti-angiogenic gene TSP-1 in CAD patients, with no significant differences in other angiogenic factors. Additionally, inhibiting TSP-1 expression significantly improved angiogenic deficiency in CAD patients.

Summary: The current guidelines recommend prophylactic implantable cardioverter-defibrillator (ICD) for patients with symptomatic heart failure (HF) and low left ventricular ejection fraction. However, not all patients will benefit from ICD treatment. This study evaluated the feasibility of using the MADIT-II-based Risk Stratification Score (MRSS) to predict the survival benefit of prophylactic ICDs in patients with HF. The results showed that different risk subgroups had varying levels of ICD survival benefit.
