Usefulness of Low-Dose Dobutamine Echocardiography to Predict Response and Outcome in Patients Undergoing Cardiac Resynchronization Therapy

A substantial proportion of patients who meet the current guidelines for cardiac resynchronization therapy (CRT) fail to respond to this pacing modality. Although appropriate patient selection and left ventricular (LV) lead location have been ascribed as determinants of CRT response, the interaction among contractile reserve, dynamics of dyssynchrony, and lead location is not well understood. The present study prospectively evaluated the effect of contractile reserve and dobutamine-induced changes in LV synchrony, in relation to the LV lead location, as predictors of the response to CRT. In the present study, 31 patients were prospectively evaluated and underwent low-dose dobutamine echocardiography. The dobutamine-induced increase in ejection fraction (contractile reserve [CR]) was measured, and the most mechanically delayed segment was identified to classify patients into 2 groups. Group 1 had a CR of > 20% and a LV lead position concordant with the mechanically delayed segment. Group 2 included the remaining patients (i.e., low CR, discordant LV lead position, or both). Patients in group 1 were significantly more likely to have an echocardiographic response at 6 months (80% of group 1 vs 29% of group 2, p = 0.018) and had an improved 2-year heart failure hospitalization-free survival rate (90% in group 1 vs 33% in group 2, p = 0.006). In conclusion, low-dose dobutamine echocardiography provides information that can help to predict responders to CRT. The response rates and heart failure hospitalization-free survival were improved in those patients with a CR > 20% and an LV lead tip concordant with the most delayed mechanical segment. Summary: This study aimed to investigate the prognostic impact of systolic pulmonary artery pressure (SPAP) in mitral annular calcium (MAC)-related mitral valve (MV) disease and determine its determinants. The findings revealed that higher SPAP was strongly associated with all-cause mortality, independent of cardiovascular risk factors and indices of MV dysfunction. The determinants of SPAP included mean transmitral gradient, mitral regurgitation severity, left ventricular ejection fraction, and moderate or severe aortic stenosis.

Summary: This study investigated the prevalence of ischemia with nonobstructive coronary arteries (INOCA) among patients with at least moderate ischemia and the relationship between ischemia severity and non-obstructive atherosclerosis severity. The results showed that the prevalence of INOCA was 13% and there was no significant association between the severity of ischemia and the severity of nonobstructive atherosclerosis. Female sex was identified as an independent predictor of INOCA.

Summary: Mitral annular calcification (MAC)-related mitral valve (MV) dysfunction is a recognized entity with high morbidity and mortality. However, there is a lack of data on the gender differences in MAC phenotype and clinical implications. This study analyzed 3,524 patients with extensive MAC and significant MAC-related MV dysfunction to determine gender differences in clinical and echocardiographic characteristics and the prognostic importance of MAC-related MV dysfunction. Women had higher transmitral gradients, more hypertrophy, and mitral regurgitation compared to men. Men had worse adjusted survival, but the prognostic impact of transmitral gradient did not differ significantly between genders.

Can GRACE Risk Score Predict Mortality and the Need for Thrombolytic Treatment in Acute Pulmonary Embolism?

Summary: This study aimed to evaluate the predictive value of the GRACE risk score in acute pulmonary embolism (PE) patients for 30-day mortality and the need for thrombolytic treatment. The study found that the GRACE risk score, along with the PESI score, were independent risk factors associated with 30-day mortality and the need for thrombolytic treatment. The GRACE risk score showed high sensitivity and specificity in predicting mortality, and its combination with the PESI score helped define high-risk PE patients and predict poor prognosis.

Summary: Transthoracic echocardiography (TTE) is commonly used to evaluate isolated tricuspid regurgitation (TR), but there is limited research comparing its quantification with magnetic resonance imaging (MRI). In this study, a novel TTE algorithm was developed to identify severe TR based on TTE parameters. The algorithm had a higher accuracy than the current guidelines' criteria in detecting severe TR by MRI.

Coronary Angiography and Chronic Total Occlusion Angioplasty in a Patient With Situs Inversus from the Right Radial Approach

Summary: This article presents a case of a patient with known situs inversus who underwent cardiac catheterization and was found to have chronic total occlusion in the right coronary artery. Situs inversus is a rare congenital abnormality characterized by the inverted position of chest and abdominal organs. Cardiac catheterization is uncommon in patients with this particular abnormality, highlighting the importance of customizing techniques to engage coronary arteries and optimize guide support for percutaneous coronary intervention if needed.

Pericardial Adipose Tissue Thrombospondin-1 Associates With Antiangiogenesis in Ischemic Heart Disease

Summary: This study evaluated the angiogenic capacity of adipose tissue in patients undergoing cardiac surgeries and found that patients with coronary artery disease (CAD) had reduced angiogenic capacity in their pericardial adipose tissue. Gene expression analysis revealed upregulated expression of the anti-angiogenic gene TSP-1 in CAD patients, with no significant differences in other angiogenic factors. Additionally, inhibiting TSP-1 expression significantly improved angiogenic deficiency in CAD patients.

Summary: The current guidelines recommend prophylactic implantable cardioverter-defibrillator (ICD) for patients with symptomatic heart failure (HF) and low left ventricular ejection fraction. However, not all patients will benefit from ICD treatment. This study evaluated the feasibility of using the MADIT-II-based Risk Stratification Score (MRSS) to predict the survival benefit of prophylactic ICDs in patients with HF. The results showed that different risk subgroups had varying levels of ICD survival benefit.
