Closure of Atrial Septal Defect With the Amplatzer Septal Occluder in Adults

Transcatheter closure of atrial septal defects (ASDs) was performed mainly in children and adolescents. Information about outcome and complications in adults was limited. From November 1997 to November 2005, percutaneous closure of ASDs using the Amplatzer septal occluder was attempted in 650 consecutive adult patients. Median patient age was 45.8 +/- 16.2 years (range 18 to 90), mean systolic artery pressure was 33.3 +/- 10.6 mm Hg (range 11 to 85), and mean pulmonary and systemic blood flow (Qp/Qs) ratio was 1.9 +/- 0.7 (range 0.8 to 6.6). Mean stretched diameter of the ASD was 21.2 +/- 5.1 mm (range 3.1 to 43). Seventy-eight patients (12%) had multiple defects. Of 572 patients with a single ASD, device implantation was successful in 563 patients (98%). During follow-up, complete closure could be achieved in 96% of patients with a single ASD and 71% of patients with multiple defects. Mean systolic artery pressure decreased to 28.3 +/- 10.1 nun Hg and mean Qp/Qs ratio decreased to 1 +/- 0.3. The 3 complications that occurred during the procedure were device embolization (2 patients; 0.3%) and transient ST depression (1 patient; 0.2%). The most common complication immediately after the procedure and during follow-up was new-onset atrial fibrillation (28 patients; 4.3%). Electrical cardioversion was successfully in most. Complications requiring emergency or elective surgery occurred in 6 patients (0.9%; hemopericardium, 2 patients, 0.3%; device embolization, 3 patients, 0.5%, and pericardial tamponade, 1 patient, 0.2%). In conclusion, closure of ASDs using the Amplatzer septal occluder in adults was efficient and safe, with excellent long-term success rates. Serious complications were rare. (c) 2009 Elsevier Inc. No significant in-hospital complications were observed.

Summary: Bone fracture healing is a complex process involving early immune reactions, bone formation, and angiogenesis. This study focuses on the effects of bone sialoprotein (BSP) immobilized in a collagen type I gel on angiogenesis. In vitro analyses and a yolk sac membrane assay demonstrate that BSP induces angiogenesis and that collagen type I is the optimal carrier for BSP. This suggests that BSP could be a promising molecule for bone tissue regeneration.

An unexpected result of a routine cardiac consultation in a patient with nephrological problems

Summary: A 17.5-year-old boy with chronic proteinuria was referred for a cardiology consultation, where an abnormal tumour-like structure measuring 1.3 x 1.5 cm in the left atrium was discovered and confirmed as myxoma. After surgery and recovery, the boy was found to have a hereditary hypercoagulability condition and was recommended to continue treatment at multiple clinics.

A giant heart tumour diagnosed accidentally in a 6-year-old boy with wide QRS complex tachycardia

Summary: The paper presents a case report of a 6-year-old boy with broad QRS tachycardia, who was accidentally diagnosed with a cardiac tumor. The tachycardia was quickly interrupted by performing a carotid sinus massage and a large tumor in the interventricular septum was discovered. The treatment included medication and cardiac monitoring, with a recommended follow-up.

Can GRACE Risk Score Predict Mortality and the Need for Thrombolytic Treatment in Acute Pulmonary Embolism?

Pericardial Adipose Tissue Thrombospondin-1 Associates With Antiangiogenesis in Ischemic Heart Disease

Summary: This study evaluated the angiogenic capacity of adipose tissue in patients undergoing cardiac surgeries and found that patients with coronary artery disease (CAD) had reduced angiogenic capacity in their pericardial adipose tissue. Gene expression analysis revealed upregulated expression of the anti-angiogenic gene TSP-1 in CAD patients, with no significant differences in other angiogenic factors. Additionally, inhibiting TSP-1 expression significantly improved angiogenic deficiency in CAD patients.

Evaluation of MADIT-II Risk Stratification Score Among Nationwide Registry of Heart Failure Patients With Primary Prevention Implantable Cardiac Defibrillators or Resynchronization Therapy Devices

Summary: The current guidelines recommend prophylactic implantable cardioverter-defibrillator (ICD) for patients with symptomatic heart failure (HF) and low left ventricular ejection fraction. However, not all patients will benefit from ICD treatment. This study evaluated the feasibility of using the MADIT-II-based Risk Stratification Score (MRSS) to predict the survival benefit of prophylactic ICDs in patients with HF. The results showed that different risk subgroups had varying levels of ICD survival benefit.
