Presence and extent of coronary calcified plaque evaluated by coronary computed tomographic angiography are independent predictors of ischemic stroke in patients with suspected coronary artery disease

Although ischemic stroke and coronary artery disease (CAD) share common risk factors and pathophysiology, the risk of stroke in patients with CAD remains unclear. We sought to evaluate the risk of ischemic stroke in patients with suspected CAD according to coronary computed tomographic angiography (CCTA) and single-photon emission computed tomography (SPECT) findings. Presence, severity, and extent of CAD were evaluated in 1137 patients with suspected CAD who underwent CCTA and SPECT. Primary outcome was the occurrence of ischemic stroke. During follow-up (median 26 months), ischemic stroke was observed in 25 patients (2.2 %). The presence of coronary plaque on CCTA was associated with the occurrence of ischemic stroke (2.8 vs. 0.6 %; p = 0.029), while the presence of PD on SPECT was not (2.0 vs. 2.3 %; p = 0.768). Stroke occurrence was not increased by the presence of significant stenosis of a parts per thousand yen50 % DS (2.8 %; p = 0.943), but was further increased by the plaque presence in a parts per thousand yen2 vessels (6.1 %; p = 0.001) or a parts per thousand yen3 segments (4.1 %; p = 0.019). Presence of calcified plaque, and calcified plaque in a parts per thousand yen2 segments were also associated with ischemic stroke occurrence (4.3 %; p < 0.001, and 5.6 %; p < 0.001, respectively) and were the independent risk factors when adjusted to age of a parts per thousand yen65, hypertension, presence of any coronary plaque and plaque in a parts per thousand yen3 segments (adjusted HR 6.09; 95 % CI 1.38-26.87; p = 0.017, and adjusted HR 5.47; 95 % CI 1.85-16.19; p = 0.002, respectively). The risk of ischemic stroke was associated with the presence and extent of coronary atherosclerotic plaque evaluated by CCTA, but not with the presence and extent of myocardial ischemia evaluated by SPECT. Especially, calcified coronary plaque presence and extent were the independent predictors of ischemic stroke and allowed further risk stratification.

Prediction of Deterioration of Left Ventricular Function Using 3-Dimensional Speckle-Tracking Echocardiography in Patients With Left Bundle-Branch Block

Summary: This study found that 2D global longitudinal strain (GLS) is associated with cardiovascular outcomes in patients with left bundle-branch block, but the predictive value of 3D speckle-tracking echocardiography has not been investigated in these patients. The study retrospectively analyzed the association between 2D and 3D strains and left ventricular ejection fraction (LVEF) in patients with preserved or reduced LVEF. The results showed that 2D-GLS is predictive of maintaining LVEF in patients with preserved LVEF, while a lower 3D-global circumferential strain is associated with persistently reduced LVEF in patients with reduced LVEF.

Long-term cardiovascular events in hypertensive patients: full report of the Korean Hypertension Cohort

Summary: This study evaluated the long-term cardiovascular complications among Korean patients with hypertension and compared them with that of controls without hypertension. The results showed that the cardiovascular event rates were higher among patients with hypertension, even in low-risk populations. Systolic blood pressure levels >= 150 mmHg were significantly associated with the occurrence of cardiovascular events.

Machine learning reveals sex-specific associations between cardiovascular risk factors and incident atherosclerotic cardiovascular disease

Summary: This study aimed to explore the differences in the association between cardiovascular risk factors and atherosclerotic cardiovascular disease (ASCVD) risk in men and women using machine learning. A random forest model was developed to predict the 10-year ASCVD probabilities in each sex. The results showed that there were significant sex-specific associations between cardiovascular risk factors and ASCVD events, with higher total cholesterol or LDL cholesterol levels being more strongly associated with the risk of ASCVD in men, while older age and increased waist circumference were more strongly associated with the risk of ASCVD in women.

Summary: The aim of this study was to investigate the prognostic utility of left ventricular global longitudinal strain (LV-GLS) in patients with hypertrophic cardiomyopathy (HCM) and a left ventricular ejection fraction (LVEF) of 50-60%. The study included 349 HCM patients with an LVEF of 50-60%, and the results showed that LV-GLS was an independent predictor of a composite outcome including cardiovascular death in these patients. Therefore, LV-GLS can aid in risk stratification in HCM patients with an LVEF of 50-60%.

Summary: This article describes the blood pressure measurement protocol and quality control program in the Korea National Health and Nutrition Examination Survey (KNHANES). KNHANES transitioned to mercury-free auscultatory devices for blood pressure measurement in 2020 and will adopt Microlife as the unified device from 2023. Under the new protocol, blood pressure measurements are conducted three times with specific conditions, and the average of the second and third readings is used as the representative value. The quality control program ensures the competence of examiners and involves weekly calibration and on-site evaluations.
