Further research in multiple areas can help improve nursing home adjustment, interventions, and health policies.

Summary: This study evaluated the prevalence of polypharmacy and hyper-polypharmacy in elderly patients in South Korea from 2010 to 2019. The results showed that the prevalence of polypharmacy remained stably high, while the prevalence of hyper-polypharmacy increased over the past 10 years. Therefore, strategies to address polypharmacy need to be implemented, and further research on the clinical outcomes associated with polypharmacy is required.

Summary: This study aimed to investigate the receipt of comprehensive medication review (CMR) among older adults and factors associated with it. The results showed that most older adults on multiple medications had not received a CMR, but many were interested in it. Furthermore, the study found that receiving a CMR was associated with taking multiple medications and food insecurity, while poor self-reported physical health was associated with a lower likelihood of receiving a CMR.

Summary: Prescribing cascades in older adults with dementia were studied using administrative databases in Nova Scotia. It was found that the combination of diuretics following CCB was the most common prescribing cascade and bladder anticholinergics following cholinesterase inhibitors the second most common. Women were more likely to be associated with the risk of bladder anticholinergics following cholinesterase inhibitors and diuretics following CCBs. However, exposure to the inciting medications did not increase the risk of exposure to the second treatments.

Summary: A multipronged approach was used to obtain the best possible medication history (BPMH) for hospitalized older adults, revealing high prevalence of medication discrepancies in the study cohort. Factors such as lower age, greater prehospital medication count, and admission from assisted living or skilled nursing facility were significantly associated with greater medication discrepancies. These findings highlight the importance of addressing medication discrepancies prior to hospital discharge in order to support safe prescribing practices for multimorbid older adults.

Summary: The European Society for Clinical Nutrition and Metabolism (ESPEN) and the European Association for the Study of Obesity (EASO) launched the Sarcopenic Obesity Global Leadership Initiative (SOGLI) to reach expert consensus on a definition and diagnostic criteria for Sarcopenic Obesity (SO). The present paper describes the proceeding of the Sarcopenic Obesity Global Leadership Initiative (SOGLI) meeting that was held on November 25th and 26th, 2022 in Rome, Italy. This consortium involved the participation of 50 researchers from different geographic regions and countries. The document outlines an agenda advocated by the SOGLI expert panel regarding the pathophysiology, screening, diagnosis, staging, and treatment of SO that needs to be prioritized for future research in the field.

Summary: Altered L-arginine metabolism and lower nitric oxide (NO) bioavailability were observed in adults with long COVID. Supplementation of L-arginine plus vitamin C significantly increased L-arginine concentrations and L-arginine/ADMA in these individuals, suggesting its potential as a remedy to improve NO bioavailability in long COVID patients.

Summary: This study aimed to compare the burden of post-COVID-19 syndrome in depressed and non-depressed patients and investigate the correlation between the burden of post-COVID-19 syndrome and the severity of mixed depression. The findings showed that patients with depression had higher burden of post-COVID-19 syndrome and the severity of mixed depression was associated with the frequency of syndrome symptoms. These findings can help clinicians choose appropriate treatment options.

In Italy, dedicated health facilities were established for the management of Long-COVID, but there is significant variation in the services provided. To address this, a multidisciplinary panel was formed to create recommendations for the clinical management of Long-COVID.

Summary: The study aimed to examine the relationship between adherence to specific exercise modalities and the severity of sarcopenia in older adults in Italy. The data was collected as part of the Longevity Check-Up 7+ (Lookup 7+) project. The results showed that engagement in running and/or swimming and strength training with or without stretching had a positive impact on reducing the severity of sarcopenia in older adults.

Summary: This study assessed the 6-month outcomes of COVID-19 hypoxemic respiratory failure patients treated with helmet noninvasive ventilation (NIV) or high-flow nasal oxygen. The results showed similar quality of life and functional outcomes for both treatment methods. Worse outcomes were associated with the need for invasive mechanical ventilation. These data suggest that helmet NIV can be safely used in hypoxemic patients.

Summary: This retrospective analysis examines the long-term impact of COVID-19 disease and finds that patients infected with COVID-19 require more chronic medication treatment. Disease severity and age are identified as the best predictors for an increase in medication usage, while anti-SARS-CoV-2 vaccination provides significant protection. Multidisciplinary and integrated care pathways should be implemented for older, frail patients and those with severe disease.

Summary: During the COVID-19 pandemic, the Radiation Oncology Department in Italy implemented an eHealth platform for televisit/monitoring/consultation to reduce patient exposure to the virus. Data collected from patients who underwent televisit/teleconsultation showed that the eHealth system was useful and user-friendly. Further research is needed to support widespread adoption.

Summary: This article describes the evidence on the usefulness of a Comprehensive Geriatric Assessment (CGA)-based approach during the COVID-19 pandemic. The multidimensional, CGA-based approach allows better identification of individual risk profiles and frailty status of infected, recovered but with a post-COVID-19 condition, and non-infected older adults. Capturing older patients' needs through CGA may offer the possibility to guide clinical decision and implement personalized medicine.
