Polypharmacy in nursing home residents with severe cognitive impairment: Results from the SHELTER Study

Objective: Pharmacological treatment of older adults with cognitive impairment represents a challenge for prescribing physicians, and polypharmacy is common in these complex patients. The aim of the current study is to assess prevalence and factors related to polypharmacy in a sample of nursing home (nursing home) residents with advanced cognitive impairment. Methods: We conducted a cross-sectional analysis of 1449 nursing home residents with advanced cognitive impairment participating to the Services and Health for Elderly in Long Term Care (SHELTER) project, a study collecting information on residents admitted to 57 nursing home in eight countries. Data were collected using the International Resident Assessment Instrument (InterRAI) for long-term care facilities. Polypharmacy status was categorized into three groups: nonpolypharmacy (zero to four drugs), polypharmacy (five to nine drugs), and excessive polypharmacy (>= 10 drugs). Results: Polypharmacy was observed in 735 residents (50.7%) and excessive polypharmacy was seen in 245 (16.9%). Compared with nonpolypharmacy, excessive polypharmacy was associated directly with ischemic heart disease (odds ratio [OR], 3.68; 95% confidence interval [CI], 2.01-6.74), diabetes mellitus (OR, 2.66; 95% CI; 1.46-4.84), Parkinson's disease (OR, 2.84; 95% CI, 1.36-5.85), gastrointestinal symptoms (OR, 1.20; 95% CI, 1.43-3.39), pain (OR, 3.12; 95% CI, 1.99-4.89), dyspnea (OR, 2.57; 95% CI, 1.31-5.07), and recent hospitalization (OR, 2.56; 95% CI, 1.36-5.85). An inverse relation with excessive polypharmacy was shown for age (OR, 0.74; 95% CI, 0.59-0.93), activities of daily living disability (OR, 0.79; 95% CI, 0.63-0.99) and presence of a geriatrician on the nursing home staff (OR, 0.36; 95% CI, 0.20-0.64). Conclusion: Polypharmacy and excessive polypharmacy are common among nursing home residents with advanced cognitive impairment. Determinants of polypharmacy status includes not only comorbidities, but also specific symptoms, age, and functional status. A geriatrician in the facility is associated with lower prevalence of excessive polypharmacy. (C) 2013 The Alzheimer's Association. All rights reserved.

Summary: The European Society for Clinical Nutrition and Metabolism (ESPEN) and the European Association for the Study of Obesity (EASO) launched the Sarcopenic Obesity Global Leadership Initiative (SOGLI) to reach expert consensus on a definition and diagnostic criteria for Sarcopenic Obesity (SO). The present paper describes the proceeding of the Sarcopenic Obesity Global Leadership Initiative (SOGLI) meeting that was held on November 25th and 26th, 2022 in Rome, Italy. This consortium involved the participation of 50 researchers from different geographic regions and countries. The document outlines an agenda advocated by the SOGLI expert panel regarding the pathophysiology, screening, diagnosis, staging, and treatment of SO that needs to be prioritized for future research in the field.

Effects of L-Arginine Plus Vitamin C Supplementation on L-Arginine Metabolism in Adults with Long COVID: Secondary Analysis of a Randomized Clinical Trial

Summary: Altered L-arginine metabolism and lower nitric oxide (NO) bioavailability were observed in adults with long COVID. Supplementation of L-arginine plus vitamin C significantly increased L-arginine concentrations and L-arginine/ADMA in these individuals, suggesting its potential as a remedy to improve NO bioavailability in long COVID patients.

Summary: The study aimed to investigate the relationship between protein intake and blood pressure, glucose levels, and blood cholesterol in a large sample of Italian older adults. The findings showed that high protein intake was negatively associated with diastolic blood pressure and positively associated with total blood cholesterol levels in a large cohort of Italian older adults.

Mixed Depression in the Post-COVID-19 Syndrome: Correlation between Excitatory Symptoms in Depression and Physical Burden after COVID-19

Summary: This study aimed to compare the burden of post-COVID-19 syndrome in depressed and non-depressed patients and investigate the correlation between the burden of post-COVID-19 syndrome and the severity of mixed depression. The findings showed that patients with depression had higher burden of post-COVID-19 syndrome and the severity of mixed depression was associated with the frequency of syndrome symptoms. These findings can help clinicians choose appropriate treatment options.

Engagement in Aerobic Exercise Is Associated with a Reduced Prevalence of Sarcopenia and Severe Sarcopenia in Italian Older Adults

Summary: The study aimed to examine the relationship between adherence to specific exercise modalities and the severity of sarcopenia in older adults in Italy. The data was collected as part of the Longevity Check-Up 7+ (Lookup 7+) project. The results showed that engagement in running and/or swimming and strength training with or without stretching had a positive impact on reducing the severity of sarcopenia in older adults.

Summary: This study assessed the 6-month outcomes of COVID-19 hypoxemic respiratory failure patients treated with helmet noninvasive ventilation (NIV) or high-flow nasal oxygen. The results showed similar quality of life and functional outcomes for both treatment methods. Worse outcomes were associated with the need for invasive mechanical ventilation. These data suggest that helmet NIV can be safely used in hypoxemic patients.

Increase in Chronic Medications and Polypharmacy-The Multifaceted Burden of COVID-19 Disease on Public Health Care

Summary: This retrospective analysis examines the long-term impact of COVID-19 disease and finds that patients infected with COVID-19 require more chronic medication treatment. Disease severity and age are identified as the best predictors for an increase in medication usage, while anti-SARS-CoV-2 vaccination provides significant protection. Multidisciplinary and integrated care pathways should be implemented for older, frail patients and those with severe disease.

Summary: During the COVID-19 pandemic, the Radiation Oncology Department in Italy implemented an eHealth platform for televisit/monitoring/consultation to reduce patient exposure to the virus. Data collected from patients who underwent televisit/teleconsultation showed that the eHealth system was useful and user-friendly. Further research is needed to support widespread adoption.

Summary: This article describes the evidence on the usefulness of a Comprehensive Geriatric Assessment (CGA)-based approach during the COVID-19 pandemic. The multidimensional, CGA-based approach allows better identification of individual risk profiles and frailty status of infected, recovered but with a post-COVID-19 condition, and non-infected older adults. Capturing older patients' needs through CGA may offer the possibility to guide clinical decision and implement personalized medicine.
