Summary: This study aimed to evaluate whether double-dose influenza vaccination during hospitalization for acute coronary syndrome (ACS) would further reduce the risk of major cardiopulmonary events. The results showed that double-dose influenza vaccination before discharge did not reduce cardiopulmonary outcomes compared with standard-dose vaccination in the outpatient setting.

The results showed that an early invasive strategy was associated with a decreased risk for all-cause mortality in these patients. Further research is needed to validate these findings.

Summary: Acute coronary syndrome (ACS) is a common cause of death in individuals older than 55 years. Young individuals with ACS (yACS) have unique epidemiologic features and require specific prediction rules to improve time-dependent prediction metrics. By exploring temporal framing structures and using subgroup-specific prediction rules, the predictive accuracy for adverse clinical events in yACS can be optimized, leading to a higher detection rate compared to using rules designed for the global cohort.

Summary: ACS in COVID-19 patients is associated with worse outcomes. This study found that COVID-19 ACS patients had more comorbidities and unfavorable clinical characteristics on admission, as well as higher rates of major complications and mortality compared to non-COVID ACS patients. Aortic regurgitation, serum creatinine levels, and respiratory failure therapy were the strongest predictors of mortality.

Summary: This study compared the costs of healthcare resource use, stroke/systemic thromboembolism, and major bleedings between non-valvular atrial fibrillation patients who initiated apixaban and other oral anticoagulants. The results showed that patients initiating apixaban had lower costs in terms of healthcare resource use and event-related costs compared to other oral anticoagulants. Apixaban may be cost-saving compared to VKAs and cheaper than other DOACs, although the cost differences are limited.

Summary: In patients with non-ST-elevation myocardial infarction (NSTEMI), total occlusion of the culprit coronary artery (OCA) is not uncommon. This study aimed to determine the frequency and clinical impact of OCA at presentation in a large population of patients with NSTEMI who underwent early invasive management. The results showed that patients with OCA had earlier coronary angiography, less involvement of the left anterior descending artery, higher prevalence of angiographic thrombus, higher mortality at 7 days, but similar mortality rates at 6 months.

Summary: This study compared the benefits of strict vs. standard control of blood pressure with the potential benefits of controlling other modifiable risk factors in patients with chronic coronary syndromes. The results showed that the benefit associated with strict blood pressure control was marginal and driven only by the diastolic component, while uncontrolled other risk factors were associated with a gradually increasing risk. Therefore, optimizing other risk factors should be prioritized over further reduction of systolic blood pressure. The optimal blood pressure target in chronic coronary syndromes may be <=139/79 mmHg.

Summary: The study aimed to characterize the long-term risk of first recurrence of ACS among survivors of an incident ACS, based on the STEMI/NSTEMI/UA diagnosis. The risk of recurrence did not depend on the type of the incident ACS, but major complications and impaired LVEF increased the risk. The most frequent forms of recurrence were NSTEMI and UA.

Age, body mass index, glomerular filtration rate, ethnicity, alcohol intake, and left ventricular ejection fraction were identified as predictors of NOAF. The study suggests that more intensive preventive measures and systematic screening for atrial fibrillation should be considered in this patient population.
