Semaglutide for weight loss and cardiometabolic risk reduction in overweight/obesity

Adriana C. Mares, Saurav Chatterjee, Debabrata Mukherjee

Summary: Obesity is associated with increased risk of cardiovascular disease. Semaglutide is a recently approved drug for chronic weight management in adults with obesity or overweight. It offers a new treatment option in conjunction with a weight management program.

High Flow Nasal Cannula Oxygenation Successfully Used as Bridge Therapy for Systemic Thrombolysis in COVID-19 Associated Intermediate-high Risk Pulmonary Embolism

Maria F. Aguilar-Piedras, Mateo Porres-Aguilar, Debabrata Mukherjee, Guillermo Cueto-Robledo, Ernesto Roldan-Valadez, Patricio A. Tapia-Vargas

Summary: The risk of venous thromboembolism in COVID-19 patients is increasing, especially in severe cases. Individualized treatment is crucial for patients with intermediate high, high-risk PE.

Optimal Treatment of Tapered Coronary Artery Lesions

Chandni Vashee, Adriana Mares, Angelica Lehker, Debabrata Mukherjee

Summary: The success of PCI is defined by post-procedure angiographic outcomes, procedural events, and longer-term clinical outcomes. Stenting long and complex lesions increases the risk of stent thrombosis and restenosis even in the long term. Tapered lesions in the target coronary artery lesion can pose challenges during PCI and affect the outcomes.

Epidemiology, Pathophysiology, and Management of Coronary Artery Disease in the Elderly

Kahtan Fadah, Aimee Hechanova, Debabrata Mukherjee

Summary: Elderly patients over the age of 75 years are highly susceptible to coronary artery disease (CAD) due to age being an important risk factor for atherosclerosis and a predictor of poorer outcomes. CAD is a major cause of mortality and morbidity in this population. Concerns about functional frailty, comorbidities, and patient preference for conservative or no treatment have led to a reduced interest in prospective studies in this high-risk group. This review provides an overview of the epidemiology, pathophysiology, and management of CAD in older adults.

Low-Dose Alteplase versus Conventional Anticoagulation to treat Submassive Pulmonary Embolism in Hispanic Patients

Emily Zientek, Kelsey Talkington, Joshua Gardner, Yi Guo, Debabrata Mukherjee, Manu Rajachandran, Tariq S. Siddiqui, Nils P. Nickel

Summary: This study aims to explore the use of low-dose tissue plasminogen activator (tPA) in Hispanic patients with submassive pulmonary embolism (PE). The results showed that compared to patients treated with heparin alone, those who received low-dose tPA had significantly shorter hospital stays. The study also found no clinically relevant bleeding complications in the low-dose tPA group.

Reduced-Dose Thrombolysis in Acute Pulmonary Embolism A Systematic Review

Adrian Rojas Murguia, Debabrata Mukherjee, Chandra Ojha, Manu Rajachandran, Tariq S. Siddiqui, Nils P. Nickel

Summary: Pulmonary embolism (PE) is a significant cause of mortality, particularly in cancer patients. Although thrombolysis has shown efficacy in treating acute PE, the optimal dosage and infusion rates have not been established. Higher doses of systemic thrombolysis come with an increased risk of major bleeding. However, evidence suggests that low-dose thrombolysis administered via a peripheral vein can effectively accelerate thrombolysis and reduce the risk of major bleeding in acute PE.

Impact of Ablation during Index Hospitalization on All-Cause Mortality in Patients with Typical Atrial Flutter: Insights from National Multicenter Database

Muhammad Amer, Frank H. Annie, Shubash Adhikari, Debabrata Mukherjee

Summary: This study compared the impact of ablation during index hospitalization (IH) on all-cause mortality (ACM) in patients with newly diagnosed typical atrial flutter (tAFL) versus patients without ablation. Using a large database, researchers found that ablation during IH was associated with a significant reduction in mortality at 6, 12, and 36 months of follow-up in patients with tAFL.

Prevalence and clinical significance of deep vein thrombosis in Hispanic patients with acute pulmonary embolism

Adrian Rojas Murguia, Swathi Prakash, Fernando Segovia, Fatih Ayvali, Michael Brockman, Sahithi Nadella, Vishwajeet Singh, Alok Kumar Dwivedi, Manu Rajachandran, Debabrata Mukherjee, Nils P. Nickel

Summary: This study retrospectively examined Hispanic patients admitted for acute pulmonary embolism (PE) and found that the presence of concomitant deep vein thrombosis (DVT) was associated with clot burden on chest CT, right heart strain, and 30-day mortality. Among the 391 patients studied, 42.9% had concomitant DVT on admission, and 9.9% died during the 30-day follow-up, with a higher mortality rate among those with concomitant DVT. Proximal DVT independently predicted 30-day mortality, even after adjusting for age, gender, and PE severity index scores. The prevalence of concomitant DVT in Hispanic patients with acute PE was relatively low, but the location of the DVT had significant prognostic relevance.
