EAACI consensus statement for investigation of work-related asthma in non-specialized centres

Work-related asthma (WRA) is a relevant problem in several countries, is cause of disability and socioeconomic consequences for both the patient and the society and is probably still underdiagnosed. A correct diagnosis is extremely important to reduce or limit the consequences of the disease. This consensus document was prepared by a EAACI Task Force consisting of an expert panel of allergologists, pneumologists and occupational physicians from different European countries. This document is not intended to address in detail the full diagnostic work-up of WRA, nor to be a formal evidence-based guideline. It is written to provide an operative protocol to allergologists and physicians dealing with asthma useful for identifying the subjects suspected of having WRA to address them to in-depth investigations in a specialized centre. No evidence-based system could be used because of the low grade of evidence of published studies in this area, and instead, key messages or suggestions are provided based on consensus of the expert panel members.

Summary: This retrospective study evaluated the prevalence of SE-IgE sensitization in a multicentric Italian cohort of severe asthmatic patients. SEB-IgE sensitization was found to be associated with female gender and comorbidities such as CRS and CRSwNP. No significant associations were observed between SEB-IgE sensitization and atopy, exacerbations, and corticosteroid dosages. Patients with SEB-IgE sensitization were younger and had an earlier disease onset, suggesting that it may be an independent risk factor for developing asthma.

Summary: The study aimed to investigate the effects of partial reboot surgery on recurrence rates, recurrence-free survival, quality of life, and oral corticosteroid intake in patients with recalcitrant CRSwNP. The results showed that the reboot approach significantly reduced recurrence rates, extended recurrence-free survival, improved quality of life, and decreased oral corticosteroid intake. This study is of great importance for the treatment of CRSwNP.

Summary: MicroRNAs (miRNAs) are increasingly being used as biomarkers for diagnosis and treatment of various diseases. This study analyzed miRNA expression profiles in serum and lung tissue from severe asthma patients with and without oral corticosteroids (OCS) treatment. MiRNA-Seq and RT-qPCR were used for evaluation and validation of miRNA levels. Altered expression of miRNAs in serum and lung tissue was observed, with hsa-miR-221-5p and hsa-miR-769-5p showing the best differentiation between OCS-treated and untreated individuals. These miRNAs were found to be associated with important signaling pathways for asthma and regulated the expression of genes involved in these pathways. The findings suggest that miRNA profiles can serve as biomarkers for OCS treatment in asthma patients.

However, NRS are prone to bias and have limited validity. This study compared the effects of AIT between RCTs and NRS and investigated the reasons for discrepancies in study results.

Effect of Obesity on the Expression of Genes Associated with Severe Asthma-A Pilot Study

Summary: Asthma is a complex condition influenced by genes and environment, and obesity is a risk factor that can worsen asthma therapy and severity. This study aimed to determine the impact of obesity on the expression of genes associated with severe asthma. Three groups of subjects were examined: non-obese asthmatics, obese asthma patients, and non-asthmatic obese individuals. The findings showed that obesity decreased the expression of MSR1 and PHLDA1, and significantly reduced CPA3 expression, while having no effect on CD86, IL-10, SERPINB2, PI3, CHI3L1, and IL-8 expression. These results suggest that obesity can modify the expression of genes related to asthma, particularly severe asthma.
