Summary: Lipidic nanoparticles, especially liposomes, have been proven to be successful and versatile for intracellular drug delivery. Recently, liposomes have shown promise in vaccination strategies and nucleic acid delivery in the COVID-19 pandemic. However, liposomes are not immunologically inert and may have limitations in certain conditions. This study investigated the physico-chemical characteristics of liposomes and their effects on basophil-like cells, revealing the association between liposome characteristics and cell activation and inflammatory responses.

Clinical and Translational Significance of Basophils in Patients with Cancer

Summary: Basophils, despite their small proportion in circulating blood leukocytes, play significant roles as potent immune effector cells. They express high-affinity receptors for IgE and store powerful inflammatory mediators ready to be secreted. They are implicated in allergic diseases, hypersensitivity reactions, chronic inflammation, and responses to infections. Despite their activation by Th2-biased signals, which are also observed in certain tumors, basophils have been neglected in cancer research. This article discusses the presence and functional significance of basophils in cancer patients and the tumor microenvironment, exploring gene expression analyses and the potential use of basophil activation test (BAT) to predict hypersensitivity to cancer treatments and monitor patient desensitization.

Principles, potential, and limitations of ex vivo basophil activation by flow cytometry in allergology: A narrative review

Summary: The major challenge in allergy diagnosis is the development of accessible and reliable diagnostics for predicting clinical outcomes after exposure to allergens. Flow-assisted analysis and the basophil activation test (BAT) show promise in meeting this requirement for various IgE-dependent allergies, but proper execution and interpretation are essential. Other uses of BAT include therapeutic monitoring and identifying allergenic recognition sites.

Diagnostic Approach of Hypersensitivity Reactions to Cefazolin in a Large Prospective Cohort

Summary: This study aimed to characterize patients reporting cefazolin allergic reactions and analyze the diagnostic role of in vivo and in vitro tests. Severe immediate-type reactions were often reported by patients allergic to cefazolin, with skin tests enabling a diagnosis in half of patients, but DPT could not be performed in all patients due to reaction severity. BAT may be a promising diagnostic tool for further research to clarify the underlying mechanisms, especially in severe reactions.

Patients who have confirmed reaction to a vaccine component should undergo further investigation to select the right vaccine for immunization. Basophil activation test can be a useful tool in identifying vaccine-reactive patients.

Venom Anaphylaxis: Decision Points for a More Aggressive Workup

Summary: Diagnostic testing for insect venom allergy requires a detailed initial history to assess severity and predict future reactions. Venom skin tests and specific IgE measurement confirm the diagnosis but have limited predictive value. Testing for serum IgE to recombinant venom component allergens can distinguish true allergy from cross reactivity. Basophil activation tests and elevated tryptase levels are useful for detecting and predicting the severity of venom allergy. Bone marrow biopsy is the definitive tool for characterizing mast cell disorders in patients with insect sting allergy. (From J Allergy Clin Immunol Pract 2023;11:2024-31)

Efficacy and safety of omalizumab in adult patients with wheat- dependent exercise-induced anaphylaxis: Reduction of in vitro basophil activation and allergic reaction to wheat

Summary: This study evaluated the efficacy and safety of long-term omalizumab treatment for patients with WDEIA. During the treatment, more than 80% of the patients achieved a basophil activation rate below 10% against all wheat preparations, and 68.8% of the patients achieved the primary endpoint of no allergic reactions after wheat product ingestion. However, the proportion of patients with a basophil activation rate below 10% decreased gradually during the observation period.

Summary: This study describes the utility of a newly developed tool for identifying patients with a high possibility of anaphylaxis and investigates the frequency of anaphylaxis with each drug during the perioperative period in Japan. The developed composite score, which includes skin tests and basophil activation tests, improved the certainty of anaphylaxis diagnosis. The top causative agents were rocuronium, sugammadex, and cefazolin.

Summary: This study explored cross-reactivity between chlorhexidine (CHX) and structurally similar compounds using a passive mast cell activation test (pMAT). Results showed that some CHX-allergic patients exhibited mast cell degranulation responses to alexidine (ALX) and octenidine (OCT), while polyhexamethylene biguanide (PHMB) directly activated mast cells via occupation of MRGPRX2.

Summary: Immediate hypersensitivity reactions are challenging to diagnose due to their wide range of clinical presentations and uncertain mechanisms. These reactions can be caused by activation and degranulation of specific and nonspecific mast cells and basophils. Degranulation of these cells leads to the release of inflammatory mediators, which can vary depending on the trigger.
