Summary: This study aimed to compare the efficacy of oral omeprazole and intravenous omeprazole in reducing rebleeding rate in peptic ulcer patients. The results suggest that oral omeprazole may be as effective as intravenous omeprazole in high-risk bleeding ulcers patients. High-quality randomized controlled trials are still necessary to further confirm these findings.

Summary: By comparing high-dose continuous infusion and non-high-dose intermittent infusion regimens, it was found that the non-high-dose intermittent infusion regimen is more effective in reducing the risk of rebleeding within 7 days, shortening the length of hospital stay, and reducing units of packed red blood cells transfusion.

Summary: The study compared the safety and efficacy of anaprazole and rabeprazole in duodenal mucosal healing, showing that anaprazole at dosages of 20 and 40 mg is effective with good safety profile in treating active duodenal ulcers, indicating the potential for advancing to a phase III clinical trial.

Summary: This study evaluated the efficacy and safety of the novel proton pump inhibitor anaprazole compared with rabeprazole in Chinese patients with duodenal ulcers. The results showed that anaprazole was non-inferior to rabeprazole in terms of efficacy, and the presence of Helicobacter pylori infection and CYP2C19 genetic polymorphism did not significantly influence the outcomes.

Summary: This study compared the clinical outcomes of marginal ulcer bleeding (MUB) with peptic ulcer bleeding (PUB). The results showed that MUB had a higher rebleeding rate and a higher mortality rate compared to PUB. Prophylactic use of proton pump inhibitors (PPIs) may reduce the risk of rebleeding in MUB.

Summary: Proton pump inhibitors are widely used for the treatment of acid-related gastrointestinal disorders and stress ulcer prophylaxis. However, their use may lead to significant complications such as fractures, Clostridium difficile infection, acute kidney injury, chronic kidney disease, and hypomagnesemia. In the hospital setting, proton pump inhibitors are often overused and inappropriately continued after discharge.

Summary: In patients with controlled PUB undergoing endoscopic treatment, second-look endoscopy with DEP-guided evaluation and treatment is safe and associated with a very low risk of rebleeding. It may be considered in high-risk patients as an alternative to DEP use during index endoscopy.

Summary: The study found that 44.4% of high-risk patients were concomitantly treated with proton pump inhibitors. While factors like living situation, educational level, and income were associated with PPI treatment in crude analyses, these associations became insignificant when adjusted for medication use, age, sex, and comorbidity. The research suggests room for improvement in ulcer prophylaxis and highlights the potential benefits of co-prescribing proton pump inhibitors to at-risk patients.

Summary: This study aimed to investigate whether eradication of Helicobacter pylori would protect against aspirin-associated peptic ulcer bleeding. The results showed a significant reduction in bleeding risk during the first 2.5 years after H. pylori eradication, but this advantage diminished with longer follow-up.

Summary: This study aimed to assess the incidence of VTE and bleeding within 30 days following major surgery in patients with ovarian cancer and evaluate the association between VTE and thromboprophylaxis duration. The study found that the cumulative incidence of VTE was considerable in patients with ovarian cancer following major surgery, but extended thromboprophylaxis did not significantly lower the risk of VTE. Overall, the importance of this article is 7 out of 10.

Summary: This study aims to compare the effects of proton pump inhibitors (PPIs) and histamine-2 receptor antagonists (H2RAs) on the gut microbiota. The results showed that PPIs had a greater impact on the gut microbiome and oral-to-gut transmission compared to H2RAs. PPI usage led to a significant increase in both the number and total abundance of oral species present in the gut, and gut microbiome-based machine learning classifiers could accurately distinguish PPI users from non-PPI users.

Summary: The treatment of refractory Helicobacter pylori infection remains challenging, and factors such as treatment length, antibiotic dosage, number of drugs, and appropriate antibiotic selection should be considered. Strategies such as extended treatment duration, higher PPI doses, four-drug therapy, and tailored therapy based on susceptibility testing can improve eradication rates.

Summary: This study quantitatively examined the gastric mucosal innervation density (MID) and its relationship with gastroparesis symptoms and gastric emptying in patients with diabetes. The study found that the gastric MID in patients with diabetes was significantly lower compared to healthy controls, and this reduction was correlated with gastroparesis symptoms and glycemic control. The study also showed prolonged gastric emptying in patients with diabetes. These findings provide important pathology and functional biomarkers for diabetic gastroparesis and shed light on the underlying pathophysiology.

Summary: This study aimed to evaluate the characteristics of the esophageal microbiome in achalasia patients and the changes before and after peroral endoscopic myotomy (POEM). The study found that there were distinct microbiome differences in achalasia patients, with an abundance of Lactobacillus. After POEM, the abundance of Lactobacillus decreased, and Neisseria increased. Further studies are needed to understand the long-term effects of these microbial changes.

Summary: This study investigates the clinical and physiological characteristics of patients with PPI-refractory laryngeal symptoms. The results show that these patients have a high prevalence of psychological distress and sleep disturbance. Moreover, patients with concurrent gastroesophageal reflux disease symptoms have more severe sleep disturbances. Therefore, management of these patients should address the psychological and sleep-related factors.

Summary: This case report describes a rare case of acute cholangitis caused by duodenal reflux without duodenal obstruction in a patient who underwent self-expandable metallic stent (SEMS) placement for obstructive jaundice. The patient presented with fever and severe abdominal pain, and fecal-impacted bowel loops were found. After several treatments, including SEMS revision and administration of metoclopramide, the symptoms were relieved. This case highlights the importance of prophylactic prokinetic therapy after SEMS placement to prevent complications.
