Methods: MDD users (>= 65 years, >= 5 oral chronic drugs) were randomly selected from eight Dutch community pharmacies. Non-MDD users (>= 5 oral chronic drugs) were matched on age and gender. Medication adherence was assessed by using the Medication Adherence Reporting Scale (MARS) and medication knowledge by asking the indication of drugs. Cognitive function was measured with Mini-Mental State Examination (MMSE) for a sub selection of patients. Results: the percentage of patients being adherent to all drugs was higher for MDD users (n = 119, 81%) compared with non-MDD users (n = 96, 58%, P < 0.001).The percentage of patients with adequate knowledge was lower for MDD users (40%) compared with non-MDD users (79%, P < 0.001). The differences in adherence were independent of knowledge and MMSE scores. Conclusion: this study shows that older patients receiving their drugs via MDD reported a higher medication adherence compared with patients receiving manually dispensed drugs, despite a lower knowledge and lower cognitive function among patients receiving MDD.

Thyroid Function and Risk of Anemia: A Multivariable-Adjusted and Mendelian Randomization Analysis in the UK Biobank

Nicolien A. van Vliet, Annelies E. P. Kamphuis, Wendy P. J. den Elzen, Gerard J. Blauw, Jacobijn Gussekloo, Raymond Noordam, Diana van Heemst

Summary: This study investigated the association between thyroid function and anemia. It found that self-reported clinical diagnosis of hypothyroidism was associated with higher prevalence of anemia, but there was no causal association with variation of thyroid function within the euthyroid range. However, intracellular regulation of thyroid hormones might play a role in the development of anemia.

HPV type-specific distribution among family members and linen in households of cutaneous wart patients

E. Ghorzang, M. N. C. de Koning, J. N. Bouwes Bavinck, J. Gussekloo, K. D. Quint, J. J. Goeman, M. C. W. Feltkamp, S. C. Bruggink, J. A. H. Eekhof

Summary: This study demonstrates the similarity of HPV types among wart cases, family members, and household linen, showing that HPV1 and HPV2 are more likely to be transmitted to family members and household linen. Dissemination of skin wart-causing HPV types, especially HPV1 and HPV2, from wart cases to household contacts and linen, is highlighted as an important finding deserving further investigation.

Declining daily functioning as a prelude to a hip fracture in older persons-an individual patient data meta-analysis

Willeke M. Ravensbergen, Jeanet W. Blom, Andrew Kingston, Louise Robinson, Ngaire Kerse, Ruth O. Teh, Rolf H. H. Groenwold, Jacobijn Gussekloo

Summary: This study found that in very old individuals, decline in daily functioning already starts before a hip fracture. Therefore, a hip fracture is an indicator of ongoing decline and full recovery should be evaluated with consideration of pre-fracture decline.

Do we AGREE on the targets of antihypertensive drug treatment in older adults: a systematic review of guidelines on primary prevention of cardiovascular diseases

Jonathan M. K. Bogaerts, Leonie M. von Ballmoos, Wilco P. Achterberg, Jacobijn Gussekloo, Sven Streit, Milly A. van der Ploeg, Yvonne M. Drewes, Rosalinde K. E. Poortvliet

Summary: Translating the available evidence into clinical guidance for primary cardiovascular prevention in older adults is challenging. This review provides an overview of the thresholds and targets of antihypertensive drug therapy for older adults in current guidelines and evaluates the relationship between guideline characteristics and recommended targets.

How progress evaluations are used in postgraduate education with longitudinal supervisor-trainee relationships: a mixed method study

Marnix P. D. Westein, A. S. Koster, H. E. M. Daelmans, M. L. Bouvy, R. A. Kusurkar

Summary: The combination of measuring performance and giving feedback can be challenging for supervisors, and there are conflicting perspectives on the effects of supervisor-trainee relationships on assessing performance. This study examines how progress evaluations are used in postgraduate education with longitudinal supervisor-trainee relationships. The findings suggest that progress evaluations are useful for discussing trainee development and adding structure to the learning process, but the reliability of scoring performance is low.

Summary: The loss of skeletal muscle function, strength, and mass is common in older adults, and it has significant socioeconomic impact. Subclinical hypothyroidism, which is prevalent in older adults, has been associated with reduced muscle strength. However, there hasn't been a randomized placebo-controlled trial to investigate the effect of treating subclinical hypothyroidism on muscle function and mass.

Summary: This meta-analysis of data from 21 cohorts across six continents found that women have a higher risk of developing dementia than men, especially in low- and lower-middle-income economies. Longer education and former alcohol use are stronger risk factors for dementia in men than in women, while other risk factors have similar effects in both genders.

Out-of-hospital opioid prescriptions after knee and hip arthroplasty: prescribers and the first prescribed opioid

Heather E. van Brug, Rob G. H. Nelissen, Frits R. Rosendaal, Liza N. van Steenbergen, Eveline L. A. van Dorp, Marcel L. Bouvy, Albert Dahan, Maaike G. J. Gademan

Summary: This study in the Netherlands examined the first prescribed opioids and the prescribers after knee and hip arthroplasty between 2013 and 2018. It found that the dose of the first prescribed opioid was associated with the total dispensed dose and long-term opioid use in the first year post-surgery. Orthopaedic surgeons primarily prescribed the first prescription, while general practitioners were responsible for consecutive prescriptions.

Patients' Perspectives on the Development of Prescription Opioid Use Disorder in Patients with Chronic Non-Cancer Pain

Lisa E. M. Davies, Ellen S. S. Koster, Katinka F. M. Damen, Harmen Beurmanjer, Arnt F. A. Schellekens, Marcel L. L. Bouvy

Summary: In the past decade, prescription opioid use has increased exponentially, leading to a rise in opioid use disorders. This study aimed to understand the patients' perspective on developing a prescription opioid use disorder (OUD). Through in-depth interviews with 25 adults undergoing treatment for prescription OUD, it was found that the development of OUD is influenced by various factors, which can be categorized into three themes: experiences driving initiation, experiences driving continuation, and experiences with prescription OUD. The findings highlight the need for better guidance, monitoring, and pain management strategies to prevent and address prescription OUD.
