Although emerging evidence indicates that the incidence of both acute kidney injury (AKI) and chronic kidney disease (CKD) in children is rising and the etiologies are dramatically changing, relatively little is currently known regarding the potential for transition from AKI to CKD. In both situations, early intervention can significantly improve the dismal prognosis. However, the lack of a uniform AKI definition and the paucity of early, predictive biomarkers have impaired our ability diagnose AKI early to institute potentially effective therapies in a timely manner. Fortunately, recent data has validated a multidimensional AKI classification system for children. In addition, the application of innovative technologies has identified candidates that are emerging as early biomarkers of both AKI and CKD. These include neutrophil gelatinase-associated lipocalin, liver-type fatty acid-binding protein, and kidney injury molecule-1. Studies to validate the sensitivity and specificity of these biomarkers in clinical samples from large cohorts and from multiple clinical situations are currently in progress, facilitated by the development of commercial tools for the reproducible measurement of these biomarkers across different laboratories. NGAL/hepcidin-25 ratio and AKI subtypes in patients following cardiac surgery: a prospective observational study

Saban Elitok, Prasad Devarajan, Rinaldo Bellomo, Berend Isermann, Michael Haase, Anja Haase-Fielitz

Summary: The study aimed to investigate the prognostic value of AKI subtypes defined by the NGAL/hepcidin-25 ratio in higher-risk cardiac surgery patients. Results showed that subclinical and combined AKI patients had significantly higher in-hospital mortality rates, indicating a strong association between these biomarker-informed AKI subtypes and prognosis.

Human Stem Cell and Organoid Models to Advance Acute Kidney Injury Diagnostics and Therapeutics

Naomi Pode-Shakked, Prasad Devarajan

Summary: Acute kidney injury (AKI) is a common and serious problem affecting people of all ages, with a lack of understanding in its pathobiology hindering the development of new diagnostics and therapeutics. Recent advancements in using human stem cells, organoids, and in silico approaches have been made in studying human AKI.

Summary: The purpose of this study is to determine risk factors for recurrent stone formation in non-ambulatory children. The study found that urinary oxalate excretion is significantly increased in these patients, while urinary calcium excretion is similar to healthy controls. Further research and dietary intervention are needed to reduce stone formation.

Summary: The Renal Activity Index for Lupus (RAIL) can non-invasively identify lupus nephritis (LN) and assess the efficacy of LN induction therapy. RAIL scores can differentiate clinically active LN from inactive LN or without LN. RAIL scores significantly decrease after complete LN remission.

Choline supplementation attenuates experimental sepsis-associated acute kidney injury

Denise C. Hasson, Miki Watanabe-Chailland, Lindsey Romick-Rosendale, Adeleine Koterba, Dashiell S. Miner, Patrick Lahni, Qing Ma, Stuart L. Goldstein, Prasad Devarajan, Stephen W. Standage

Summary: This study found that altered choline metabolism is involved in sepsis-associated acute kidney injury (SA-AKI) in both humans and mice, and choline administration improved renal function in experimental SA-AKI. These findings suggest that mouse models can help in investigating clinically relevant mechanisms and that choline supplementation may ameliorate SA-AKI in humans.
