Radical ablative surgery and radial forearm free flap (RFFF) reconstruction for patients with oral or oropharyngeal cancer: postoperative outcomes and oncologic and functional results

Conclusions. Radical ablative surgery and radial forearm free flap (RFFF) reconstruction provide promising oncologic and functional results in patients with oral or oropharyngeal cancer. Objectives. To assess the postoperative outcomes and the oncologic and functional results, with their main predictive factors, after radical ablative surgery and RFFF reconstruction for patients with oral or oropharyngeal cancer. Patients and methods. Between 2000 and 2006, we prospectively analyzed the postoperative, oncologic and functional outcomes of all previously untreated patients who underwent this type of surgery. Results. A total of 132 patients were enrolled in this study. There were three RFFF failures. The rate of surgical complications was 20%. The 5-year locoregional control and overall survival rates were 68% and 52%, respectively. Advanced age, high comorbidity index, elevated overall stage and tumoral involvement of the inner part of the cheek were correlated with a lower overall survival rate. A good functional result was obtained for oral diet, speech, mouth opening and aesthetic outcome in 87%, 80%, 86% and 88% of the patients, respectively. High comorbidity index, large flap surface, radiotherapy and tumoral involvement of the mobile tongue were significant predictors of poorer functional or aesthetic outcomes.

Docetaxel-Cisplatin-Fluorouracil Induction Chemotherapy for Larynx Preservation in Patients with Locally Advanced Hypopharyngeal Cancer: Predictive Factors of Oncologic and Functional Outcomes

Summary: This study aimed to evaluate the clinical outcomes and predictive factors in locally advanced hypopharyngeal cancer patients undergoing induction chemotherapy-based larynx preservation program. The results showed three- and five-year overall survival rates of 67% and 54%, respectively. T4 tumor stage was the only predictive factor of poor response to chemotherapy. Multivariate analysis revealed that T stage = 4, fewer cycles of chemotherapy, and lack of response to chemotherapy were significantly associated with worse overall survival. Low body mass index before therapy and enteral nutrition during therapy were significantly associated with the severity of dysphagia after treatment.

A Novel Approach of Combining Methylene Blue Photodynamic Inactivation, Photobiomodulation and Oral Ingested Methylene Blue in COVID-19 Management: A Pilot Clinical Study with 12-Month Follow-Up

Summary: This study evaluated the effectiveness of local methylene blue photodynamic viral inactivation (MB-PDI) combined with orally ingested methylene blue (MB) and photobiomodulation (PBM) for treating COVID-19. The treatment showed promising results in reducing viral load and improving symptoms, with no significant adverse effects and prevention of long COVID symptoms and reinfection.

Summary: Nutritional support is crucial for tolerating and completing OPSCC treatment, but the impact of HPV status on nutritional support is debated. This study aimed to evaluate the use of RFT and its prognostic factors in OPSCC patients undergoing radiotherapy. A multicentric retrospective study included all OPSCC patients treated from 2009 to 2014. Multivariate analysis showed that tongue base involvement and concurrent chemotherapy were significantly associated with the use of RFT. HPV status was not a prognostic factor for enteral nutrition during radiotherapy for OPSCC.

Summary: This study compared the survival outcomes of salvage vs. primary total laryngectomy in patients with locally advanced laryngeal or hypopharyngeal carcinoma and determined their predictive factors. The results showed that salvage TL had significantly worse survival rates than primary TL. The predictive factors identified here should be considered in the therapeutic decision-making, especially for salvage TL patients.
