Summary: Chronic rhinosinusitis with nasal polyps (CRSwNP) has been considered a benign, chronic inflammatory, and hyperplastic disease. Recent studies have shown that autoimmune-related mechanisms play a role in nasal polyp pathology. Innate and adaptive immune pathways are mobilized during polyp formation, involving activated plasma cells, eosinophils, basophils, innate type 2 lymphocytes, mast cells, and proinflammatory cytokines in polyp tissue. The discovery of autoantibodies further supports the autoimmune nature of nasal polyps. This study elaborates on the role of autoimmunity in the pathogenesis of CRSwNP, discussing microbiome-mediated mechanisms, abnormal host immunity, and genetic changes.

Summary: This study revealed significant differences in sinus microbiome between healthy control subjects and CRSwNP patients. Microbiota community diversity was significantly lower in NECRSwNP samples compared to healthy control subjects. The abundance of pathogenic bacteria varied between ECRSwNP and NECRSwNP patients, with a significantly higher abundance of S. aureus in NECRSwNP patients.

Summary: Chronic Rhinosinusitis with Nasal Polyps (CRSwNP) is a multifactorial disease, and bacterial superantigens such as Staphylococcus aureus are known to play a role in its development. Specifically, Staphylococcus aureus nasal colonization and IgE response to Staphylococcus aureus enterotoxins have been found in CRSwNP patients. Among the enterotoxins, enterotoxin B (SEB) has been identified as a key player in the pathogenesis of CRSwNP by disrupting mucosal integrity and triggering inflammatory responses. SEB activates Toll Like Receptor 2, leading to the production of pro-inflammatory cytokines, and induces cellular stress and oxidative damage. It also promotes eosinophil degranulation and localized eosinophilic inflammation. Furthermore, SEB may be involved in the regulation of immune cells and maintenance of sinonasal mucosal inflammation. While in vitro and animal studies have provided valuable insights, further research is needed to fully understand the role of SEB in CRSwNP.

Summary: This study aimed to evaluate the efficacy of mepolizumab in patients with chronic rhinosinusitis with nasal polyps (CRSwNP) in real-life settings. A retrospective observational study was conducted on severe CRSwNP patients treated with mepolizumab. The results showed significant improvements in symptoms, polyp scores, blood eosinophils, and corticosteroid use, leading to enhanced health-related quality of life in patients with severe CRSwNP, regardless of the presence of asthma or aspirin exacerbated respiratory disease (AERD).

Summary: Chronic rhinosinusitis (CRS) is a common inflammatory disease affecting the nasal cavity and paranasal sinuses, often accompanied by nasal polyps. This study investigated the expression patterns of antioxidative enzymes and immune defense factors in the nasal mucosa and polyps of CRSwNP patients. The results showed differential gene expression in different tissues and correlation with disease severity. Surgical and corticosteroid treatment improved the appearance of the sinuses but had varying effects on gene expression patterns.
