More accurate component alignment in navigated total knee arthroplasty has no clinical benefit at 5-year follow-up

Background and purpose Computer navigation in total knee arthroplasty is somewhat controversial. We have previously shown that femoral component positioning is more accurate with computed navigation than with conventional implantation techniques, but the clinical impact of this is unknown. We now report the 5-year outcome of our previously reported 2-year outcome study. Methods 78 of initially 84 patients (80 of 86 knees) were clinically and radiographically reassessed 5 (5.1-5.9) years after conventional, image-based, and image-free total knee arthroplasty. The methodology was identical to that used preoperatively and at 2 years, including the Knee Society score (KSS) and the functional score (FS), and AP and true lateral standard radiographs. Results Although a more accurate femoral component positioning in the navigated groups was obtained, clinical outcome, number of reoperations, KSS, FS, and range of motion were similar between the groups. Interpretation The increased costs and time for navigated techniques did not translate into better functional and subjective medium-term outcome compared to conventional techniques.

Residual stability of the distal radioulnar joint following ulnar styloid fracture: influence of the remnant distal radioulnar ligaments

Deep-Learning-Based Segmentation of the Shoulder from MRI with Inference Accuracy Prediction

Current concepts in chronic traumatic anterior shoulder

Summary: Chronic traumatic anterior shoulder instability refers to recurrent trauma-associated shoulder instability that requires assessment of three anatomic lesions: a capsuloligamentous and/or labral lesion, anterior glenoid bone loss, and a Hill-Sachs lesion. Surgical treatment is usually necessary, but there is controversy regarding how to evaluate risk factors to determine the appropriate procedure. Patient risk factors for recurrence include age, hyperlaxity, and participation in competitive, contact, and overhead sports. Trauma-related factors include soft tissue lesions and bone loss, which have implications for treatment. Different treatment options are discussed and compared based on complications, return to sports parameters, short- and long-term outcomes, and the risk of osteoarthritis. Arthroscopic Bankart and open Latarjet procedures have a steep learning curve, and the development of osteoarthritis is associated with the number of previous dislocations and surgical techniques. Latarjet-type procedures have the lowest rate of dislocation recurrence and, when performed correctly, do not appear to increase the risk of osteoarthritis.

Chronic posttraumatic shoulder instability

Summary: Chronic posttraumatic shoulder instability is a trauma-related condition characterized by recurrent dislocations. Surgical treatment, such as arthroscopic Bankart repair (ABR), Latarjet, or alternative procedures, is usually needed. Risk factors including patient age, hyperlaxity, sports profile, and bone loss should be considered when deciding on the surgical approach. ABR provides high patient satisfaction and return to sports, but there is a risk of recurrent dislocations, especially in the presence of risk factors. Latarjet or bone block procedures offer sustainable stability and can be used as a revision procedure, but they can also be used as a primary treatment for chronic instability, depending on the risk factors.
