Who does it first? The uptake of medical innovations in the performance of thrombolysis on ischemic stroke patients in Germany: a study based on hospital quality data

Background: Since 2000, systemic thrombolysis has been the only approved curative and causal treatment for acute ischemic stroke. In 2009, the guidelines of the German Society for Neurology were updated and the therapeutic window for performing thrombolysis was extended. The implementation of new therapies is influenced by many factors. We analyzed the factors at the organizational level that influence the implementation of thrombolysis in stroke patients. Methods: The data published by the majority of German hospitals in their structured quality reports was assessed. We calculated a regression model in order to measure the influence of hospital/department-level characteristics (e.g., teaching status, ownership, location, and number of hospital beds) on the implementation of thrombolysis in 2006 (this is the earliest point in time that can be analyzed on this data basis). In order to measure the effect of the guideline update in 2009 on the thrombolysis rate (TR) change between 2008 and 2010, we performed a Wilcoxon signed-rank test and utilized a regression model. Results: In 2006, 61.5% of a total of 286 neurology departments performed systemic thrombolysis to treat ischemic strokes. The influencing factors for the use of systemic thrombolysis in 2006 were the existence of a stroke unit (+) and a hospital size of between 500 and 1,000 beds (-). A significant increase of the mean departmental TR (thrombolysis rate) from 6.7% to 9.2% between 2008 and 2010 was observed after the guideline update in 2009. For the departments performing thrombolysis in 2008 and 2010, our analysis could not show any additional influencing factors on a structural level that would explain the TR rise during the period 2008-2010. Conclusions: Because ischemic stroke patients benefit from systemic thrombolysis, it is necessary to examine possible barriers at the organizational level that hinder the implementation. Our data shows that, organizational factors have an influence on the implementation of thrombolysis. However, the recent guideline update resulted in a TR rise that occurred at all hospitals, regardless of the measured structural conditions, as our analysis could not identify any structural factors that might have influenced the TR after the guideline update.

Transformational Leadership and Employees' Psychological Wellbeing: A Longitudinal Study

Summary: Managers play a key role in creating a humane work environment. Transformational leadership is associated with increased employee awareness of task outcomes and values, but its impact on psychological wellbeing varies. Based on longitudinal data from a German company, this study found that transformational leadership and gender had a significant impact on future transformational leadership, while psychological wellbeing, social capital, and age did not. However, psychological wellbeing and social capital significantly influenced future psychological wellbeing. Therefore, companies should invest in social capital and consider gender aspects in the workplace, and conduct mental risk assessments to identify and address potential issues.

Patients with isolated REM-sleep behavior disorder have elevated levels of alpha-synuclein aggregates in stool

Summary: This study used surface-based fluorescence intensity distribution analysis (sFIDA) to detect and quantify single particles of alpha-synuclein aggregates in stool samples of 94 PD patients, 72 isolated rapid eye movement sleep behavior disorder (iRBD) patients, and 51 healthy controls. The results showed significantly elevated concentrations of alpha-synuclein aggregates in stool samples of iRBD patients compared to controls (p = 0.024) or PD patients (p < 0.001). The study suggests that measuring alpha-synuclein aggregates in stool using the sFIDA assay could support the diagnosis of prodromal synucleinopathies.

Differentiation of a Falcine Meningioma From Cerebral Venous Sinus Thrombosis Using DOTATATE PET

Summary: Differentiating brain tumors from nonneoplastic lesions using conventional MRI may be challenging. Clinical symptoms often remain unspecific, and imaging findings from MRI may be inconclusive. PET with the radiolabeled somatostatin receptor ligand DOTATATE was performed, and it facilitated the diagnosis of a falcine meningioma consistent with its value for the differential diagnosis of meningioma.

Summary: This study investigated the impact of tumor development and multimodality therapy on health-related quality of life (HRQoL) in glioma patients. The results showed that right hemisphere gliomas were associated with less favorable outcomes in most functional domains except communication ability. This relationship was mainly observed in T2/FLAIR lesions involving structural and functional networks in the right hemisphere.

Status epilepticus and benzodiazepine treatment: Use, underdosing and outcome-insights from a retrospective, multicentre registry

Summary: This study revealed that most patients with status epilepticus (SE) were not effectively treated with benzodiazepines (BZDs) according to current guidelines. Sufficient dosing of BZDs was found to be beneficial for patients with generalised convulsive SE, but not for other types of SE. Treatment decisions guided by the semiology of SE are crucial, and further evidence is urgently needed for the management of non-generalised convulsive SE.

Subjective and Objective Cognitive Deficits in Patients with Post-COVID Syndrome A Challenge for Neuropsychologists

Summary: Cognitive impairment is a common symptom of post-COVID syndrome (PCS), but there is inconsistency between subjective cognitive complaints (SCC) and objective results. This study investigated this discrepancy and found a 40% difference between subjective reports and test results. Patients with SCC and objective impairment showed elevated fatigue and reduced quality of life. The study suggests that more patients will likely request neuropsychological assessments even after mild infections.

Reducing uncertainty in evidence-based health policy by integrating empirical and theoretical evidence: An EbM plus theory approach

Summary: To reduce decisional uncertainty in urgent situations, a new approach called the EbM+theory approach is proposed, which integrates empirical and theoretical evidence in a context-sensitive way. This approach aims to provide decision-makers with urgent and sound recommendations based on evidence-based medicine principles. Two roadmaps are provided within this framework, one for simple interventions and the other for complex interventions, each consisting of multiple steps including applying theory, conducting mechanistic studies, and conducting experiments.

Seizures at stroke onset: A case-control study

Accelerometric Classification of Resting and Postural Tremor Amplitude

Summary: Clinical rating scales for tremor have limitations, and reliable quantitative approaches are needed. This study investigated the use of accelerometry metrics to objectively classify tremor amplitude and compared them to clinical ratings. The results showed that accelerometry can reliably quantify tremor amplitude, with finger-worn accelerometry performing slightly better.

Transcranial Direct Current Stimulation Reverses Stroke-Induced Network Alterations in Mice

Summary: In this study, the researchers investigated whether transcranial direct current stimulation (tDCS) can alter the network changes caused by cerebral ischemia and whether functional network parameters can predict the therapeutic efficacy of tDCS. The results showed that tDCS reversed the increased connectivity and reduced characteristic path length caused by ischemia. Additionally, early network changes and the network configuration prior to stroke predicted motor recovery.
