Summary: This study aimed to assess the safety of COVID-19 vaccines in patients with clonal mast cell disorders. The results showed that COVID-19 vaccines seem to be safe in these patients, including those with prior anaphylaxis following vaccination.

Summary: Historically, understanding of human mast cells has lagged behind other cell lineages, and there is currently no identified pharmacological agent that can completely and selectively inhibit human MC function. Mast cell diseases are categorized as intrinsic or extrinsic to the MC compartment, with mediators playing a key role in MC activation disorders.

Summary: The systematic review aims to present proposed theories of pathogenesis for idiopathic anaphylaxis (IA), discuss its classification, diagnostic approach, and management. Recent findings show that IA represents a major diagnostic challenge and certain conditions such as mastocytosis and red meat allergy may overlap with its symptomatology. Patients with IA should always carry an epinephrine autoinjector for prompt treatment and prophylaxis.

Summary: This study found that increased copy number variants of the TPSAB1 gene are associated with nonclonal elevated tryptasemia and mastocytosis. Patients with mastocytosis have a higher proportion of HaT+ cases, which have specific clinical characteristics and disease burdens. Therefore, knowledge of HaT status may be useful for personalized management of patients with mastocytosis.

Summary: Diagnosis of anaphylaxis should involve comprehensive consideration, including detailed history and objective findings. Using diagnostic criteria and considering rare triggers, as well as evaluating for mast cell disorders, can aid in the diagnosis. Referral to specialists may be necessary.

Summary: Idiopathic anaphylaxis refers to anaphylaxis without a known cause despite comprehensive allergic workup. The diagnostic approach involves ruling out common and uncommon allergens and other conditions that may mimic anaphylaxis. Idiopathic anaphylaxis is more prevalent in adults and the epidemiology has decreased in recent years due to increased knowledge and the discovery of new clinical entities. Activation of mast cells plays a pivotal role in anaphylaxis, therefore, disorders such as mastocytosis, mast cell activation syndrome, and alpha-tryptasemia should also be excluded as they may present with similar symptoms.

Summary: The diagnosis of MCAS relies on typical clinical signs and symptoms, rapid increase in tryptase level, and response to antimediator therapy. The Spanish Network on Mastocytosis score serves as a predictive model for selecting MCAS candidates for bone marrow studies based on the probability of an underlying clonal mast cell disorder.

Summary: This study found that only 2% of patients with suspected MCAS were confirmed to have the condition, suggesting that MC activation may not be the main driver of symptoms in most patients with suspected MCAS. The study also revealed a high prevalence of comorbid depression and anxiety in these patients.

Summary: Using an insurance claims database and a retrospective study at the University of Michigan, it was found that the prevalence of HVA in the US is lower than previously reported, while the prevalence of mastocytosis in patients with HVA is higher but still lower than previously reported. There was no correlation between tryptase level and reaction severity in patients undergoing VIT.

Summary: Research reveals that restoring cellular NAD+ concentration by supplementing NAD+ precursors can suppress mast cell activity in allergic reactions, thus alleviating the allergic response. This therapeutic strategy may hold significant importance for limiting mast cell-mediated anaphylactic responses.

Summary: Mast cell activation plays a crucial role in allergic reactions, inflammatory states, and mast cell activation syndromes. While there is currently no treatment approach to eradicate mast cells, long-term use of drugs that inhibit KIT function may lead to a decrease in tissue mast cells and improvement in symptoms. For patients with KIT D816V-positive mastocytosis, effective KIT inhibitors like avapritinib may be used for mast cell eradication, but potential side effects need to be considered.

Summary: The study investigates the effects of cannabidiol (CBD) on mast cell degranulation and passive cutaneous anaphylaxis in mice. CBD suppresses antigen-stimulated mast cell degranulation by inhibiting downstream signaling molecules and calcium mobilization. In addition, CBD inhibits the development of mast cells and suppresses passive cutaneous anaphylaxis in mice.

Summary: MCAS and mastocytosis are two disorders related to mast cell activation, with the former characterized by massive systemic activation of mast cells and the latter by clonal accumulation of mast cells in various organ systems. Mast cells are crucial effector cells in allergic diseases, and proper diagnosis and management are essential for patients with mastocytosis.

Summary: Mast cells are not only IgE effectors of anaphylaxis, but also defenders against infections and toxins. They play a role in inflammation regulation and tissue repair but are also involved in allergies and various pathological conditions. Mast cell activation syndrome (MCAS) has been proposed to explain symptoms caused by mast cell activation, but there is still debate about the diagnostic criteria.
