New Normative Database of Inner Macular Layer Thickness Measured by Spectralis OCT Used as Reference Standard for Glaucoma Detection

Purpose: This study examines the capacity to detect glaucoma of inner macular layer thickness measured by spectral-domain optical coherence tomography (SD-OCT) using a new normative database as the reference standard. Methods: Participants (N = 148) were recruited from Leuven (Belgium) and Zaragoza (Spain): 74 patients with early/moderate glaucoma and 74 age-matched healthy controls. One eye was randomly selected for a macular scan using the Spectralis SD-OCT. The variables measured with the instrument's segmentation software were: macular nerve fiber layer (mRNFL), ganglion cell layer (GCL), and inner plexiform layer (IPL) volume and thickness along with circumpapillary RNFL thickness (cpRNFL). The new normative database of macular variables was used to define the cutoff of normality as the fifth percentile by age group. Sensitivity, specificity, and area under the receiver operating characteristic curve (AUROC) of each macular measurement and of cpRNFL were used to distinguish between patients and controls. Results: Overall sensitivity and specificity to detect early-moderate glaucoma were 42.2% and 88.9% for mRNFL, 42.4% and 95.6% for GCL, 42.2% and 94.5% for IPL, and 53% and 94.6 degrees/o for RNFL, respectively. The best macular variable to discriminate between the two groups of subjects was outer temporal GCL thickness as indicated by an AUROC of 0.903. This variable performed similarly to mean cpRNFL thickness (AUROC = 0.845; P = 0.29). Conclusions: Using our normative database as reference, the diagnostic power of inner macular layer thickness proved comparable to that of peripapillary RNFL thickness. Translational Relevance: Spectralis SD-OCT, cpRNFL thickness, and individual macular inner layer thicknesses show comparable diagnostic capacity for glaucoma and RNFL, GCL, and IPL thickness may be useful as an alternative diagnostic test when the measure of cpRNFL shows artifacts.

Ten-year prevalence of rhegmatogenous retinal detachment in myopic eyes after posterior chamber phakic implantable collamer lens

The natural history of untreated glaucoma is not well understood, although reducing intraocular pressure has been proven effective in slowing disease progression. Understanding the rate of progression and conversion to advanced stages of glaucoma can guide management decisions.

Normal tension glaucoma: A dynamic optical coherence tomography angiography study

Summary: This study utilized dynamic OCTA examination to compare differences between healthy individuals and glaucoma patients, as well as to distinguish subcategories of glaucoma (NTG and POAG). The results confirmed a decrease in retinal vessel density in glaucoma patients and established the necessity to correct for gender, age, and MAP. However, replication in a larger population is needed to determine whether OCTA is suitable for dynamically evaluating the retinal microvasculature.

A randomized clinical trial comparing three fixed combinations of bimatoprost with timolol in patients with open-angle glaucoma or ocular hypertension

Summary: This study compared the efficacy and safety of two fixed combination, preservative-free eye drops (bimatoprost 0.01% in combination with either timolol 0.1% or 0.5%) in a gel formulation, with bimatoprost 0.03%/timolol 0.5% in patients with open-angle glaucoma (OAG) or ocular hypertension (OHT). The results suggest that the preservative-free ophthalmic formulation of T4030a (bimatoprost 0.01%/timolol 0.1%) can be regarded as a useful tool in the therapeutic management of OAG and OHT.

Prescription trends for preservative free glaucoma medication in a public health system

Summary: The purpose of this study was to analyze the prescribing trends for glaucoma medications in a tertiary hospital and its health area in Madrid, Spain, between 2013 and 2020. The results showed that prostaglandin analogues were the most commonly prescribed drugs for monotherapy during the study period. Fixed combinations of topical hypotensives showed an upward trend and became the most dispensed drugs in 2020. The study also found a shift towards preservative-free eye drops, which replaced preservative-containing treatments in all pharmacological groups.

Basal evaluation and rates of progression based on visual fields in six different glaucoma types of a large population

Summary: The purpose of this study is to analyze the distribution of visual field mean defect (MD) in different subgroups of glaucoma patients and their progression over time. A total of 1036 visual fields were assessed in glaucoma subtypes including open-angle glaucoma, angle-closure glaucoma, congenital glaucoma, ocular hypertension, pseudoexfoliative glaucoma, and pigmentary glaucoma. The study found that congenital glaucoma and pigmentary glaucoma had the worst baseline MD and that congenital glaucoma showed an aggressive progression. The findings emphasize the importance of special attention to the management of congenital glaucoma.

Artificial Intelligence Software for Diabetic Eye Screening: Diagnostic Performance and Impact of Stratification

Summary: The MONA.health artificial intelligence screening software showed good performance in detecting referable diabetic retinopathy (DR) and diabetic macular edema (DME). The software achieved high sensitivity and specificity on both private test set and publicly available datasets. Subgroup analysis revealed slightly lower predictive values for individuals above the age of 65 and Caucasians.

Summary: A clinical and genetic study was conducted to investigate the correlation between genotype and phenotype in pediatric patients with optic atrophy 1 (OPA1) mutations. Eleven children with confirmed OPA1 mutations were identified, with reduced visual acuity being the main complaint. Most patients had a positive family history of optic atrophy. The study found various mutations in the OPA1 gene, including one that has not been previously reported.
