Impaired Wnt5a signaling in extravillous trophoblasts: Relevance to poor placentation in early gestation and subsequent preeclampsia

Background: Defective decidual endovascular trophoblast invasion and subsequent impaired spiral artery remodeling is highly associated with the pathogenesis of preeclampsia (PE). Since there are scant and conflicting data regarding the function of Wnt5a signaling in extravillous trophoblasts (EVT), the aim of this study was to investigate whethere impaired Wnt5a signaling affects the invasive and tube forming capabilities of EVT. Methods: Expression levels of Wnt ligands were compared between first trimester chorionic villi of women who later developed PE and women with unaffected pregnancies using publicly available microarray data (GSE12767). Wnt5a expression was examined in placentas using quantitative RT-PCR, Western blot analysis and immunohistochemistry. The function of Wnt5a signaling in EVT was investigated in an immortalized first trimester EVT cell line, HTR-8/SVneo, using small-interfering RNAs, recombinant human Wnt5a (rhWnt5a), and inhibitors of JNK or PKC. Results: Microarray data analysis of the first trimester placentas showed that, among Wnt ligands, Wnt5a expression was significantly lower in women who later developed PE. The mRNA and protein expression levels of Wnt5a were significantly decreased in PE placentas compared with normal term placentas. Wnt5a knockdown significantly suppressed invasion and tube formation of HTR-8/SVneo cells, while the addition of rhWnt5a augmented the cell migration, invasion, and tube formation. Repression of Wnt5a/PKC signaling in HTR-8/SVneo cells inhibited cell invasion, but did not alter cell tube formation. In contrast, inhibition of Wnt5a/JNK signaling attenuated rhWnt5a-induced invasion and tube formation capabilities. Conclusions: These findings suggest that impaired Wnt5a signaling is associated with poor placentation and subsequent PE.

Summary: This study found that the incidence of pregnancy-associated cervical cancer is increasing in Japan, with most early-stage patients successfully giving birth, although there are cases of preterm birth. Pregnant women with stage IB1 cervical cancer have the option to continue pregnancy with chemotherapy or undergo cervical removal. Treatment decisions should consider cancer stage and benefits for both mother and unborn child.

Summary: This study presents a case of uterine adenosarcoma without sarcomatous overgrowth but with TP53 mutation that exhibited clinically aggressive behaviors. The patient had an ATM mutation, which is a gene associated with homologous recombination deficiency, and exhibited a good response against platinum-based chemotherapy and possible therapeutic target by poly(ADP-ribose) polymerase inhibitors.

Obstetric pelvimetry by three-dimensional computed tomography in non-pregnant Japanese women: a retrospective single-center study

Summary: This study used three-dimensional computed tomography (CT) to investigate the accurate size of the pelvis in 1263 Japanese women of childbearing age and obtained reference values. The results showed that the mean obstetric conjugate and transverse diameter were larger than those in the survey in 1972. These data will be used as standard pelvic measurement values for the Japanese population in clinical practice.

The prognosis of neonatal respiratory status within three years after birth in chronic abruption-oligohydramnios sequence

Summary: This study aimed to examine respiratory function and risk factors for respiratory prognosis in infants with chronic abruption-oligohydramnios sequence (CAOS) within 3 years after birth and clarify the management of CAOS. The results showed that amniotic fluid volume could predict the use of inhaled nitric oxide (iNO) in neonates to improve oxygenation, but it did not affect the child's respiratory function after the newborn period. Almost all children born to women with CAOS can improve their respiratory function as they grow up.

Summary: Recent studies have shown that clones carrying common cancer mutations frequently evolve in normal tissues, but we still lack knowledge about the additional driver events that occur before these clones evolve into cancer. In this study, using phylogenetic analyses, we found unique evolutionary histories in breast cancers with der(1;16), a common driver alteration. The timing of early evolutionary events was estimated and showed that both cancer and non-cancer clones evolved from a common ancestor in the patient's early 30s. Multiple independent cancer founders from non-cancer ancestors were also observed, contributing to intratumor heterogeneity. These findings provide new insights into the evolution of breast cancer.

Characteristics of pregnancy complicated with type 1 and type 2 diabetes

Summary: Diabetes in pregnancy is a major risk factor for adverse perinatal outcomes such as congenital anomalies, hypertensive disorders of pregnancy (HDP), and macrosomia. This study aimed to explore the difference in perinatal outcomes between type 1 and type 2 diabetes groups. Materials and methods: We retrospectively reviewed 22 pregnancies with type 1 diabetes and 15 pregnancies with type 2 diabetes and analyzed maternal diabetes conditions, neonatal birthweight, and blood glucose levels. Continuous glucose monitoring and continuous subcutaneous insulin injection were also examined for type 1 diabetes pregnancies.

Is there a need for a technical certification system for gynecological robotic surgery? Questionnaire survey of members of the Japan Society of Gynecologic and Obstetric Endoscopy and Minimally Invasive Therapy

Summary: The Japan Society of Gynecologic and Obstetric Endoscopy and Minimally Invasive Therapy (JSGOE) implemented certification systems for laparoscopic surgeons and facilities in 2002 and 2014, respectively. The opinions of JSGOE members on the necessity of certification training and technical certification for robotic surgery in Japan were examined through questionnaires. Results showed that around 50% of respondents believed both systems were necessary, especially among younger generations and those with more experience in robotic surgery. However, opinions on establishing a certification system for robotic surgery were divided. Further public surveys are needed to make a final decision.
