Maternal Serum B Cell activating factor in hypertensive and normotensive pregnancies

Objectives: The objective of this study was the analysis of B-Cell Activating Factor (BAFF) levels in pregnancies affected by PE, and in pregnancies affected by fetal growth restriction without Hypertensive disorders and its possible correlation with pulse wave velocity and cardiac output. Study design: Prospective study of 69 women at 24-40 weeks gestation. Haemodynamic function was assessed in those with Pre-eclampsia (PE, n = 19), fetal growth restriction (FGR, n = 10) and healthy pregnancies (n = 40). Maternal venous BAFF levels at recruitment were measured using ELISA. We analysed the relationship between BAFF and cardiac output (CO), and BAFF and PWV (pulse wave velocity); the gold standard for assessing arterial stiffness. PWV was measured with an oscillometric device and CO using inert gas rebreathing technique. PWV and CO were converted to gestation adjusted indices (z scores). Main outcome measures: The association between BAFF levels in PE and FGR, and the relationship of BAFF with PWV and CO. Results: BAFF was higher in PE (p = 0.03) but not in FGR (p = 0.83) when compared to healthy pregnancies. There was a positive correlation between BAFF levels and z score PWV (r = 0.25, p = 0.04), but not CO (r = -0.01, p = 0.91). BAFF levels did not change with gestational age. (r = 0.012, p = 0.925). Conclusions: These findings provide evidence of a possible contribution of BAFF to both maternal inflammation and arterial dysfunction associated with PE. Though no relationship was found with another disorder of placentation: normotensive FGR, this condition is not thought to be associated with maternal inflammation.

SARS-CoV-2 infection testing at delivery: a clinical and epidemiological priority

During the study period, 3.1% of pregnant women tested positive for SARS-Cov-2, a significantly higher proportion than estimated by health authorities. Among the positive cases, 44.9% had symptoms or reported close contact with positive patients.

Evaluation of fetal cardiac function in pregnancies with well-controlled gestational diabetes

Summary: This study found that fetal right ventricular filling time was significantly shorter in pregnancies affected by gestational diabetes, indicating that fetal right cardiac function may be affected even when the diabetes is well controlled. Left ventricular filling time was also shorter in the GD group, although the difference was not significant.

Predictors of Postpartum Depression among Italian Women: A Longitudinal Study

Summary: Postpartum depression is a common issue for mothers worldwide and is associated with anxiety disorders and parenting stress. This study aimed to describe the psychological status of mothers up to 12 months postpartum and identify predictors of depressive symptoms at 12 months postpartum. The findings highlight the importance of stable programs for mothers in the first year postpartum, and ensuring a positive childbirth experience to reduce potential long-term consequences.

Summary: This study found a higher prevalence of placental maternal vascular malperfusion in HDP-FGR and early FGR groups, which were associated with abnormal feto-placental Doppler parameters. In contrast, HDP-AGA fetus and late FGR presented more heterogeneous placental lesions not severe enough to cause feto-placental Doppler anomalies. These findings suggest potential preventive nutritional approaches for pregnant women with predisposing factors for HDP-AGA fetuses and late FGR.

Maternal AA/EPA Ratio and Triglycerides as Potential Biomarkers of Patients at Major Risk for Pharmacological Therapy in Gestational Diabetes

Summary: This study aimed to assess the impact of nutraceutical supplements on patients with gestational diabetes mellitus (GD) and to explore anthropometric, metabolic, and inflammatory parameters as biomarkers for identifying individuals requiring pharmacological treatment. The results showed no significant differences in metabolic, inflammatory, and antioxidant parameters between the group receiving nutraceutical supplements and the placebo group. However, triglyceride concentration and AA/EPA ratio appear to be biomarkers for higher inflammatory levels and patients with GD who require pharmacological treatment.

Umbilical vein blood flow: State-of-the-art

Summary: Placental blood supply to the fetus can be measured by evaluating the umbilical vein blood flow. Despite its potential application in healthcare, the umbilical vein blood flow volume is still used only in research setting. One of the reasons is a concern regarding its reproducibility, partly due to technology issues. Nowadays, technology improvements make this evaluation accurate and reproducible. The aim of this review is to refresh basic elements of the physiology of umbilical vein blood flow and its analysis. Its evaluation in normal and abnormal fetal growth is also discussed.

Umbilical Vein Blood Flow in Uncomplicated Pregnancies: Systematic Review of Available Reference Charts and Comparison with a New Cohort

Summary: The objectives of this study were to review the available umbilical vein blood flow volume (UV-Q) reference ranges in uncomplicated pregnancies and compare them with UV-Q values obtained from a local cohort. Literature in English was reviewed according to PRISMA guidelines. Original articles were grouped based on UV sampling sites and formulae used to compute UV-Q. The UV-Q values were derived from formulae or tables presented by authors. A prospective observational study of uncomplicated singleton pregnancies was conducted to compare UV-Q with the systematic review results. Overall, there was a substantial heterogeneity in reported UV-Q central values, but values overlapped when using the same sampling methodology and formulae. The study suggests that UV-Q assessment is accurate and reproducible when adhering to the same methodology, encouraging further investigation on clinical applications.
