This article summarizes recent knowledge and clinical advances in machine perfusion (MP) of thoracic organs. MP of thoracic organs has gained much attention during the last decade. Clinical studies are investigating the role of MP to preserve, resuscitate, and assess heart and lungs prior to transplantation. Currently, MP of the cardiac allograft is essential in all type DCD heart transplantation while MP of the pulmonary allograft is mandatory in uncontrolled DCD lung transplantation. MP of thoracic organs also offers an exciting platform to further investigate downregulation of the innate and adaptive immunity prior to reperfusion of the allograft in recipients. MP provides a promising technology that allows pre-transplant preservation, resuscitation, assessment, repair, and conditioning of cardiac and pulmonary allografts outside the body in a near physiologic state prior to planned transplantation. Results of ongoing clinical trials are awaited to estimate the true clinical value of this new technology in advancing the field of heart and lung transplantation by increasing the total number and the quality of available organs and by further improving recipient early and long-term outcome.

Endovascular transatrial stenting of pulmonary vein stenosis after lung transplantation

Summary: Pulmonary vein stenosis (PVS) and pulmonary vein occlusion (PVO) are rare complications after lung transplantation (LTx) with limited treatment options and a high risk of graft loss. We describe two cases of successful endovascular transatrial stenting following double LTx. Both patients, one with chronic obstructive pulmonary disease and the other with idiopathic pulmonary arterial hypertension, underwent endovascular transatrial dilatation and stenting to treat PVS and PVO respectively, avoiding the need for surgical reintervention. This treatment option appears to be effective and safe for these life-threatening complications after LTx.

Hyperthermic Intrathoracic Chemotherapy (HITHOC) for Pleural Disseminated Thymoma: A Systematic Literature Review

Summary: This review provides an overview of the current literature on hyperthermic intrathoracic chemotherapy (HITHOC) for thymoma with pleural dissemination (TPD). The results suggest that combining cytoreductive surgery and HITHOC is feasible and safe for TPD, but there is significant heterogeneity in the literature, calling for further research to better understand the additional benefit of HITHOC in TPD treatment.

Summary: This study provides new insights into the abundance, expression patterns, and interactomes of endothelial cell (EC) subtypes in COVID-19 and idiopathic pulmonary fibrosis (IPF). The findings suggest the importance of ECs in the pathophysiology of both conditions and may contribute to future investigations into the progression and treatment of COVID-19 and IPF.

Computational modeling reveals inflammation-driven dilatation of the pulmonary autograft in aortic position

Summary: The pulmonary autograft in the Ross procedure is prone to failure due to dilatation caused by tissue degradation and maladaptation triggered by higher mechanical loads. The study presents a model for tissue growth and remodeling of the pulmonary autograft using a mixture theory and equations for mass turnover mediated by immune and mechanical factors. The outcomes of the model suggest that inflammation plays an important role in the autograft dilatation over time.

Matched comparison of decellularized homografts and bovine jugular vein conduits for pulmonary valve replacement in congenital heart disease

Summary: Bovine jugular vein conduits (BJV) and decellularized pulmonary homografts (DPH) are two commonly used options for pulmonary valve replacement (PVR) in congenital heart disease. Comparison of patients receiving DPH and BJV for PVR showed that DPH had a higher survival rate after 10 years, lower rates of endocarditis, explantation, and degeneration. In a subgroup analysis, DPH showed 20% higher freedom from explantation and degeneration compared to BJV. Decellularized pulmonary homografts are superior to bovine jugular vein conduits in PVR based on 10-year results.

Summary: This observational study investigated the effectiveness of SARS-CoV-2 vaccines and breakthrough infections in transplant recipients. It found significant variations in antibody response rates and rates of severe infections among different transplant types, which were modulated by specific risk factors. These findings highlight the importance of tailored COVID-19 prevention measures for transplant recipients.

The hemodynamic interplay between pulmonary ischemia-reperfusion injury and right ventricular function in lung transplantation: a translational porcine model

Summary: Lung transplantation is a challenging procedure and can result in severe damage to the transplanted pulmonary graft. The interaction between right ventricular function and lung injury during the transplantation process is poorly understood. Through a translational porcine model, it was found that right ventricular failure can occur as a result of forcing the entire cardiac output through a lung suffering from ischemia-reperfusion injury. The study also revealed a delicate balance between the development of pulmonary ischemia-reperfusion injury and right ventricular function, highlighting the potential benefits of extracorporeal life support technology in these cases.

Urgent lung volume reduction surgery is effective for secondary pneumothorax in emphysema patients

Summary: Secondary pneumothorax due to emphysema is a life-threatening condition that often requires surgery. In this case, we used LVRS to not only remove part of the lung but also close the fistula. The patient experienced improved lung function and quality of life after urgent and elective LVRS surgeries, with resolution of the air leak.
