Laparoscopic excision of deep rectovaginal endometriosis in BSGE endometriosis centres: a multicentre prospective cohort study

Objective To estimate the effectiveness and safety of laparoscopic surgical excision of rectovaginal endometriosis. Design A multicentre, prospective cohort study. Setting 51 hospitals accredited as specialist endometriosis centres. Participants 5162 women of reproductive age with rectovaginal endometriosis of which 4721 women had planned laparoscopic excision. Interventions Laparoscopic surgical excision of rectovaginal endometriosis requiring dissection of the pararectal space. Main outcome measures Standardised symptom questionnaires enquiring about chronic pelvic pain, bladder and bowel symptoms, analgesia use and quality of life (EuroQol) completed prior to surgery and at 6, 12 and 24 months postoperatively. Serious perioperative and postoperative complications including major haemorrhage, infection and visceral injury were recorded. Results At 6 months postsurgery, there were significant reductions in premenstrual, menstrual and non-cyclical pelvic pain, deep dyspareunia, dyschezia, low back pain and bladder pain. In addition, there were significant reductions in voiding difficulty, bowel frequency, urgency, incomplete emptying, constipation and passing blood. These reductions were maintained at 2 years, with the exception of voiding difficulty. Global quality of life significantly improved from a median pretreatment score of 55/100 to 80/100 at 6 months. There was a significant improvement in quality of life in all measured domains and in quality-adjusted life years. These improvements were sustained at 2 years. All analgesia use was reduced and, in particular, opiate use fell from 28.1% prior to surgery to 16.1% at 6 months. The overall incidence of complications was 6.8% (321/4721). Gastrointestinal complications (enterotomy, anastomotic leak or fistula) occurred in 52 (1.1%) operations and of the urinary tract (ureteric/bladder injury or leak) in 49 (1.0%) procedures. Conclusion Laparoscopic surgical excision of rectovaginal endometriosis appears to be effective in treating pelvic pain and bowel symptoms and improving health-related quality of life and has a low rate of major complications when performed in specialist centres.

Perspectives on modes of labour and delivery after different modalities of surgery for bowel endometriosis

Summary: The study investigated the impact of different surgical interventions for bowel/intestinal endometriosis on the mode of labor and delivery in women, revealing a lack of consensus and wide practice variations among clinicians. Findings showed that around 70% of clinicians do not counsel patients on delivery options pre-surgery, yet almost one-third would recommend a planned caesarean section if the vagina is opened. Further research is needed to assess risk factors for vaginal delivery or caesarean section in these patients, and guidance from professional societies would be beneficial for patient counseling and informed consent.

Despite the restoration of normal services, there continues to be a negative impact on benign gynaecology surgical training. Future training needs to incorporate innovative ways of acquiring surgical skills and provide pastoral support.

Therefore, addressing vaccine hesitancy among midwifery staff is essential to improve vaccine uptake in the pregnant population.

Summary: This study developed and validated a novel index (SPI) to support the strengthening of local elective surgical systems and address growing backlogs. The findings showed that hospitals with a higher SPI were associated with an increased planned surgical volume ratio, independent of income status, COVID-19 burden, and hospital type. Annual self-assessment of surgical preparedness is recommended to identify areas for improvement and enhance the resilience of local surgical systems.

Laparoscopic myomectomy to facilitate laparoscopic resection of a bleeding interstitial ectopic pregnancy

Women's experiences of birth and birth options counselling after laparoscopic or open myomectomy

Summary: There is emerging evidence that vaginal birth after open and laparoscopic myomectomy may be safe in many pregnancies. A retrospective questionnaire survey of women who had undergone myomectomy followed by pregnancy revealed that a significant percentage of women felt inadequately involved in the decision-making process for their birth plan. The majority of women expressed a preference for vaginal birth in future pregnancies.

To Fight or to Flee? A Systematic Review of Ectopic Pregnancy Management and Complications During the Covid-19 Pandemic

Summary: This study examined the impact of the COVID-19 pandemic on the management of ectopic pregnancy. The results showed that the risk of ruptured ectopic pregnancy and complications was lower in units with an Early Pregnancy Unit (EPU) structure, while the risk was higher in units without EPU structures. This suggests that EPU structures contribute to prompt diagnosis and safe management of ectopic pregnancy.
