Engineered Healing of Avascular Meniscus Tears by Stem Cell Recruitment

Meniscus injuries are extremely common with approximately one million patients undergoing surgical treatment annually in the U.S. alone. Upon injury, the outer zone of the meniscus can be repaired and expected to functionally heal but tears in the inner avascular region are unlikely to heal. To date, no regenerative therapy has been proven successful for consistently promoting healing in inner-zone meniscus tears. Here, we show that controlled applications of connective tissue growth factor ( CTGF) and transforming growth factor beta 3 (TGF beta 3) can induce seamless healing of avascular meniscus tears by inducing recruitment and step-wise differentiation of synovial mesenchymal stem/progenitor cells (syMSCs). A short-term release of CTGF, a selected chemotactic and profibrogenic cue, successfully recruited syMSCs into the incision site and formed an integrated fibrous matrix. Sustain-released TGF beta 3 then led to a remodeling of the intermediate fibrous matrix into fibrocartilaginous matrix, fully integrating incised meniscal tissues with improved functional properties. Our data may represent a novel clinically relevant strategy to improve healing of avascular meniscus tears by recruiting endogenous stem/progenitor cells.

Prospective Randomized Controlled Clinical Trial Comparing Hyperosmolar Saline to Standard Isotonic Irrigation Fluid for Arthroscopic Knee Surgery: Initial Clinical Outcomes

Summary: This study aimed to evaluate the initial clinical outcomes associated with the use of a hyperosmolar irrigation solution in patients undergoing arthroscopic knee surgery. The results showed that the hyperosmolar saline irrigation fluid did not have any detrimental effects on the surgical procedure, postoperative pain, or periarticular fluid extravasation, and it contributed to a reduction in initial postoperative narcotic pain medication consumption.

The Missouri Osteochondral Preservation System Is Associated With Better Short-Term Outcomes Than Standard Preservation Methods When Performing Osteochondral Allograft Transplantation Using Shell Grafts for Patellofemoral Lesions

Summary: This study aimed to compare the outcomes of whole-surface osteochondral allograft (OCA) transplantation using Missouri Osteochondral Preservation System (MOPS) and standard preservation (SP) methods for the treatment of patellofemoral joint lesions. The results showed that the success rate and patient-reported outcome scores of OCA transplantation using MOPS preservation method were significantly better than those using SP preservation method. Unipolar patella OCA transplantation was superior to unipolar trochlea and bipolar patellofemoral OCA transplantation in reducing pain and improving patient-reported outcomes.

Functional biomechanical comparison of Latarjet vs. distal tibial osteochondral allograft for anterior glenoid defect reconstruction

Orthopaedic trauma patient non-adherence to follow-up visits at a level 1 trauma center serving an urban and rural population

Summary: This retrospective study examined key demographic and psychosocial factors associated with non-adherence to follow-up appointments for patients with orthopaedic trauma in an urban and rural population. The study found that male sex, age between 26 and 35 years, non-white race, disabled employment, low household income, low education level, lack of insurance or Medicaid coverage, and being single were associated with higher rates of no-show appointments.

Outcomes after Anatomic Double-Bundle Posterior Cruciate Ligament Reconstructions Using Transtibial and Tibial Inlay Techniques

Summary: Surgical reconstruction is recommended for symptomatic posterior cruciate ligament (PCL) deficiency. This study compared clinical outcomes following PCL reconstruction using two tibial fixation techniques, transtibial (TT) and tibial inlay (TI), based on body mass index, comorbidities, and primary versus revision surgery. Results showed similarly favorable functional, social, and psychological outcomes for both techniques, with slightly lower pain scores in the TT cohort.

Serial ultrasonographic imaging can predict failure after meniscus allograft transplantation

Summary: This study aimed to assess the use of serial ultrasonographic imaging in predicting short-term failure after meniscus allograft transplantation (MAT). The results showed that ultrasonographic assessment at 6 months after surgery can effectively determine the risk for short-term failure. Abnormal echogenicity, persistent effusion, and extrusion with weightbearing were associated with a higher likelihood of failure.

Evaluation of Serum and Urine Biomarker Panels for Developmental Dysplasia of the Hip Prior to Onset of Secondary Osteoarthritis

Summary: This study evaluated serum and urine biomarker panels for their ability to differentiate between individuals with healthy hips and those with developmental dysplasia of the hip (DDH) before the development of secondary hip osteoarthritis (OA). The results showed that multiple urine and serum biomarker panels could effectively differentiate individuals with DDH from healthy-hip controls in a population at risk for developing secondary hip OA. These findings suggest the potential clinical application of these biomarker panels for cost-effective and timely screening for DDH in at-risk populations.

Characterizing Osteochondral Allograft Biomechanics for Optimizing Transplant Success: A Systematic Review

Evaluating the Role and Impact of Health Literacy for Patients Undergoing Knee Surgery

Summary: Health literacy refers to the ability to obtain, process, and understand basic medical information and services, and to use this knowledge to improve health. While research has mainly focused on the readability of educational materials, the impact of health literacy on patient-reported outcomes in orthopaedic surgery remains unclear. This review aimed to evaluate the existing literature on health literacy and knee surgery outcomes. A comprehensive literature search was conducted, resulting in the inclusion of six articles that highlighted the influence of general and musculoskeletal health literacy on patient expectations, outcomes, and satisfaction before and after knee surgery. However, further research is needed to determine effective strategies for addressing health literacy as a barrier to optimal patient care in orthopaedics.

Arthroscopic Hip Labral Reconstruction With Fresh Meniscal Allograft

Summary: The acetabular labrum plays a crucial role in maintaining the functional health of the hip joint. When labral insufficiency occurs due to injury, overuse, developmental disorders, or failed primary labral repair, labral reconstruction becomes necessary. Although there are various graft options available for hip labral reconstruction, there is currently no established gold standard. The development of an arthroscopic technique using fresh meniscal allograft tissue aims to create a graft that closely resembles the native labrum in terms of geometry, structure, mechanical properties, and durability.

Evaluation of mechanistic serum and urine biomarkers for secondary osteoarthritis associated with developmental dysplasia of the hip

Summary: This study aimed to determine measurable differences in mechanistic urine and serum biomarkers in patients with developmental dysplasia of the hip (DDH) before and after the diagnosis of secondary hip osteoarthritis (OA), compared to controls. Urine and serum samples were collected and analyzed for protein biomarkers. The results showed differential expression of interleukin and degradation-related proteins in DDH-related hip status before and after the diagnosis of hip OA, as well as significant differences in MCP-1 and TIMP-1 between younger and older patients in the control cohorts.

Biomechanical comparison of nitinol compression staples versus fully threaded lag screws for talonavicular arthrodesis

Heparin conjugation and CD44 modification improved the healing of lubricin-coated meniscus injuries.
